Solicitation Number: VA241-12-Q-0028 Notice Type: Combined Synopsis/Solicitation Synopsis: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA241-12-Q-0028 and is issued as an RFQ. (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular (FAC) 2005-55, effective February 2, 2012. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) The proposed contract action is for supplies or services for which the Government intends to solicit competitive offers. Interested persons may submit offers, and all eligible offers received prior to April 6, 2012, at 1:00 p.m. will be considered by the Government. The NAICS code is 339113 and the size standard is 500 Employees. This requirement is 100% set aside for Service Disabled Veteran Owned Small Business concerns. (v) This requirement consists of the following line items: One (1) each Two-Door Lead Lined Storage Cabinet for Nuclear Medicine Department (REFER TO STATEMENT OF WORK FOR COMPLETE SPECIFICATIONS AND REQUIREMENTS) (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: 1.0 INTRODUCTION The vendor shall provide all requested deliverables described in the statement of work (SOW), except as may otherwise be specified. The contractor shall provide all the equipment, material and labor to provide and deliver a standard lead lined storage cabinet as described herein. 2.0 BACKGROUND The nuclear medicine program at the Medical Center requires lead lined cabinet equipment for the safe and responsible storage of unit dose radioactive materials. 3.0 SCOPE Vendor shall provide a standard storage cabinet with two adjustable shelves and plenty of space to store radioactive material or decay radioactive waste. Each of the shelves must have a maximum capacity of 100 lbs., Double Doors, seamless stainless steel countertop work area with a 4" backsplash and a ½" spill restricting lip around the perimeter of the work surface. Lead lined thickness of ½". Heavy gauge steel hinges for durability. Heavy duty locks for security. White enamel finish. Exterior dimensions of 40" height x 36 ½" width x 24 3/4" depth. Self weight of the cabinet not to exceed 2,200 lbs. Cabinet to be a Standard Storage Cabinet manufactured by Capintec or approved equivalent. Scope to include rigging and delivery to install the cabinet in the final position of the user room inside the VA Medical Center Manchester (Building 1, Room E01). Performance Period: The period of performance is thirty days or better after the notice of award. Delivery of goods at the government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (C.O.). Normal working hours are Monday thru Friday, 7:00 AM to 3:30 PM. Place of Performance: The product will be delivered to the nuclear medicine through coordination at the warehouse at the VA Medical Center located at 718 Smyth Road in Manchester New Hampshire in Building #1, Room E01. 4.0 Submittals The vendor will submit a delivery schedule and product literature for review upon notice of award. (vii) Delivery FOB Destination to: Department of Veterans Affairs VA Medical Center 518 Smyth Road Manchester NH 03104 (viii) 52.212-1 Instructions to Offerors - Commercial Items, apply to this solicitation. (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror who is deemed the lowest bidder technically acceptable. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offeror's are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE SUBMITTED QUOTE, unless offeror has a current ORCA certification.
