- Department Veterans Affairs (VA) Consolidated Mail Out-Patient Pharmacy (CMOP) Murfreesboro TN VA-797M-12 -Q - 0134 posting FedBizOps for Pharmaceutical Supplies. RFQ is 15 line items - NDC numbers Listed Alternate NDC# can be provide by contractor Non-designated Countries not accepted: IE - Chine, Croatia, India Exception to this rule occur when no other Designated Country can provide needed pharmaceutical. Country of Origin needs to filled. VENDORS SHOULD BID ON ITEMS THAT HEY HAVE IN STOCK OR ARE ABSOLUTELY SURE THEY WILL BE ABLE TO GET THE PRODUCT IN TIME FOR DELIVERY TO CMOP. CMOP PREFERS NO SUBSTITUTES FOR INDICATED NDC'S, HOWEVER WE WILL CONSIDER SUBS ON A CASE BY CASE BASIS, IF A REQUESTED NDC IS NOT AVAILABLE FROM ANY SOURCE. ITEMS ORDERED WERE SELECTED BY GIP, AND ARE BASED ON PRE-DETERMINED LEVELS AND ORDERING POINTS WHICH ARE ESTABLISHED FROM HISTORICAL USAGE. VENDOR NEEDS TO VERIFY THE COUNTRY OF ORIGIN ON ALL NDCs SUBMITTED FOR BID FOR EVALUATION. Drug List Spreadsheet Quotes (Excel) submitted via Email to Julian.Lake@va.gov (preferred).
