- Required Clauses Provision 1552.209-99 Fill-in SOW Lab Accreditation Targets and Analytes 52.212-3 Reps and Certs Lab Accreditation IAW ISO/IES 17025 The Environmental Protection Agency (EPA) Region 1, New England, is issuing this Request for Quote (RFQ) for a one (1) year plus one (1) option year Laboratory Accreditation implementing ISO/IES 17025 requirements for accreditation as a testing laboratory in environmental chemistry. The EPA Region 1 laboratory is located at 11 Technology Drive, North Chelmsford MA. 01863-2431 and is currently accredited under the 2003 National Environmental Laboratory Accreditation Conference (NELAC) for the analytes and methods listed in Attachment A herein. The ISO/IES 17025 accreditation process must be conducted by an authorized Accredited Body (AB) having recognition as a signatory to the International Laboratory Accreditation Cooperation (ILAC) Mutual Recognition Agreement (MRA). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. Prior to award offers must be registered in the Central Contractor Registration (CCR). Registration can be made in CCR by visiting http://www.ccr.gov. The NAICS code for this requirement is 813920. This acquisition is not set aside for small business. FAR Clauses for this solicitation are included in attachment 2 herein document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-57. The ISO/IES 17025 requirements for laboratory accreditation shall include the target analytes and methods listed in Attachment A herein. Certificate of Accreditation is required prior to August 1, 2012 The Following requirements apply to this Request for Quote (RFQ): 1. This document 2. Attachment 1 Statement of Work 3. Offerors shall provide a Firm Fixed Price quote for a one (1) year plus one (1) option year of Laboratory Accreditation as specified herein. Please include the items listed in the description below in your quote. Your response is for a Firm Fixed Price award including all services and travel. Please include period of acceptance which must be until April 30, 2012 as a minimum. Item Discription 001 Year 1 Accreditation (including all services and travel) 002 Year 2 (Option) Accreditation (including all services and travel) 003 Accreditation Milestone and need dates for all documentation 004 Technical description of the items being offered Criteria and Basis for Selection for Award In accordance with FAR 13.106 an award will be made to the schedule contractor that can provide the best value. Best value shall consist of vendor meeting all requirements as stated in this Request for Quote (RFQ) and attached Statement of Work (SOW) to technically qualify. After technical acceptance lowest quoted total firm fixed price. Offerors who are not documented as authorized Accredited Body (AB) having recognition as a signatory to the International Laboratory Accreditation Cooperation (ILAC) Mutual Recognition Agreement (MRA) or do not provide the requested information will not be considered for award. The Government may contact Offerors to clarify information to better understand their quotes or to request information that appears to be inadvertently omitted. 52.212-1 -- Instructions to Offerors -- Commercial Items. Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show - (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Price and any discount terms; (6) "Remit to" address, if different than mailing address; (7) A completed copy of the representations and certifications in attachment 3 herein as stated in FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of Solicitation Amendments; Offerors shall send quotes via email to stachowski.robert@epa.gov no later than 8:00 PM (EST) March 30, 2012. Quotes received after the required date and time will not be considered for award.
