PLEASE READ THIS ENTIRE NOTICE CARFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis /solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6 under FAR Part 13 Simplified Acquisition Procedures and supplemented with additional information included in this notice. This announcement constitutes the only solicitation for VA 248-12-Q-1045, issued as a Request for Quotes (RFQ), quotes are being requested and a written solicitation will not be issued. This notice and incorporated provision and clauses are those in effect through the current Federal Acquisition Circular. The North American Industrial Classification System (NAICS) code for this procurement is 238130 with a small business size standard of $14M. This requirement is a small business set-aside and only qualified offerors may submit quotes. The contractor shall provide annual preventive maintenance (PM) service for each eighty-three (83) automatic door or closure on the attached document for the Lake City VA Medical Center, 819 S. Marion Ave, Lake City, Florida The contract performance period consists of: One base period, and three optional one year periods. (Base Period: Date of Award - 30 Sep 2012). All communication regarding quotes, questions, etc. shall be submitted by email to Linda J. Baldwin, (linda.baldwin1@va.gov). Phone calls will not be accepted.
