Solicitation Number: VA241-12-Q-0486 Notice Type: Combined Synopsis/Solicitation Synopsis: ***Please note: The original RFQ for this requirement was posted under RFQ #VA241-12-Q-0212. This is the same requirement and solicitation. The RFQ number needed to be updated for this posting due to a posting error with original number. The requirement is being posted for an additional 5 days*** (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA241-12-Q-0486 and is issued as an RFQ. (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular (FAC) 2005-55, effective February 2, 2012. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as an unrestricted requirement. The associated North American Industrial Classification System (NAICS) code for this procurement is 621511 with a small business size standard of $13.50M. (v) This requirement consists of the following line item: 01- ZIO Patch long-term cardiac rhythm monitor /Arrhythmia monitoring services (brand name or equal specifications) - Quantity: 150 ea (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: VISN 1 Contracting is requesting quotes for a one-time firm-fixed price purchase of (150 EA) ZIO Patch long-term cardiac rhythm monitor and Arrhythmia monitoring service manufactured and monitored by IRhythm Technologies. Quotes for brand name or equal products that meet or exceed the specifications listed below will also be accepted and evaluated. The closing date for this Fedbid RFQ is March 15, 2012 @ 10:00 AM EST. The items will be shipping to Boston VA Healthcare System, West Roxbury Campus, 1400 VFW Parkway, West Roxbury MA, 02132. The evaluation criteria for this RFQ will be based on the following factor, lowest price technically acceptable per FAR 15.101-2. The submitted quotes will be evaluated based on the following technical components and specifications listed below in the Product Information/Salient Characteristics section. C&A requirements do not apply and a Security Accreditation Package is not required. Please see attachment with information regarding information security and a Business Associate Agreement (BAA), both of which apply to this solicitation and requirement. Please include the following information in your quotes. 1.Name of long-term cardiac rhythm monitor with specifications (include brochures and reference material if available) 2.Detailed information regarding monitoring services. 3.Firm-fixed pricing for (150 EA) ZIO Patch long-term cardiac rhythm monitors and Arrhythmia monitoring services (Brand name or equal products quoted must meet or exceed the product information/salient characteristics listed below): 4.If your company is exempt from a (BAA) because you are a HIPAA healthcare provider and Covered Entity, please provide documentation for review. Product Information/Salient Characteristics The Zio Patch is a long-term cardiac rhythm monitor that provides continuous monitoring for up-to-14 days (significantly longer than the time period of a typical Holter). By providing a longer time period of continuous recording, the Zio Patch improves the likelihood of capturing arrhythmias and provides for an equal or higher diagnostic yield versus other devices on the market. Diagnostic findings are not reported until the device is returned to Rhythm Technologies and will be posted to their website for review and printing for final interpretation by Cardiologist. The Zio Patch is indicated for use on patients who may be asymptomatic or who may suffer from transient symptoms such as palpitations, dizziness, light-headedness, presyncope and syncope, shortness of breath, anxiety and fatigue. As with all iRhythm products, the Zio Patch is 100% recyclable. No part of the device will end up in a landfill, minimizing the impact of disposable devices on the environment. Intended Use: The ZIO Patch will be applied to upper left chest of patient and to be worn up to 14 days to monitor patient's rate and rhythm. It is a diagnostic tool to aid physicians in the treatment of their patients with arrhythmia problems or to rule out arrhythmia problems due to various symptoms. Benefits: oEasy for patient to use: Apply and remove device from chest after monitoring period.
