Sources Sought Notice Title: Provide Technical Consultation Services to OQM in Support of Survey Development for the Design and Collection of Survey Data via Web General Information: This Sources Sought Notice (SS) is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Office of the Director, OD and O ffice of Research Services, ORS. Purpose: The purpose of this Sources Sought Notice is to identify qualified small business concerns including 8(a), HUBZone, or Service-Disabled Veteran-owned business concerns that are interested in and capable of providing the services described herein. OD does not intend to award a contract on the basis of responses received nor otherwise pay for the preparation of any information submitted. This market survey/sources sought announcement is not a request for proposals and the Government is not committed to award a contract pursuant to this announcement. The information from this market research is for planning purposes only and will assist the Government in planning its acquisition strategy. This is strictly market research and the Government will not entertain questions concerning this market research. As a result of this Sources Sought Notice, the OD may issue a Request for Quotation (RFQ). THERE IS NO SOLICITATION AVAILABLE AT THIS TIME. However, should such a requirement materialize, no basis for claims against OD shall arise as a result of a response to this Sources Sought Notice or the OD's use of such information as either part of our evaluation process or in developing specifications for any subsequent requirement. Statement of Work: Background The Office of Quality Management (OQM) assists the National Institute of Health's (NIH) Office of Research Services (ORS) with a variety of organizational development and improvement initiatives, including the implementation of systematic customer assessment methods. As part of OQM's technical support provided to numerous ORS customers, OQM designs, gathers, and analyzes survey data to assess customers' perceptions of ORS service delivery and seek ideas for improvement. Purpose The purpose of this effort is to provide a technical solution to OQM in support of the design and collection of survey data via the web. Scope of Task The objectives of this project are: •1. To assist OQM in designing surveys using survey software •2. To provide technical advice on survey design, data collection and analysis as requested by OQM •3. To conduct actual data analysis tasks in support of data gathered using survey software products as requested by OQM Evaluation Criteria •1. Experience with adapting survey software products to federal government agencies; •2. Experience in providing technical consultation to Federal government service providers using survey software products; •3. Developing customer satisfaction surveys tailored to specific organizational needs; •4. Providing raw data from survey results; •5. Further analysis as requested Deliverables The contractor shall: Deliver pilot survey files as requested by OQM Modify survey files as needed based on feedback from OQM Deliver data analysis results in a final report as requested and specified by OQM Provide raw data from the survey results to OQM Provide further analysis as requested by OQM Location of Work to be Performed The contractor shall perform the work at the contractor's work site unless otherwise specified by the Government. T o the extent possible and to minimize cost, the contractor will communicate with the Project Officer by telephone and email. Travel and per diem will not be reimbursed. Government-furnished Property N/A Security The contractor shall not directly or indirectly use, or allow use of, data obtained through or in connection with this task which has not been made available to the general public. The contractor agrees not to disclose data or analysis results from this project to any person outside the Government, nor to any Government employee or official, unless authorized by the Office of Quality Management. Records Management As a Federal contractor you are required to maintain records in accordance with two primary records-related codes: The Federal Records Act of 1950, which provides the basic overall framework for how Federal records are to be managed. Title 36 of the Code of Federal Regulations ( CFR ), which provides specific details on how records are managed in the Federal Government. For recordkeeping guidelines, see CFR Title 36, Paragraph 1222.48--Data created or received and maintained for the Government by contractors The definition of a Federal record refers to materials "regardless of physical form" and those "preserved or appropriate for preservation." A record includes all books, papers, maps, photographs, machine-readable materials, or other documentary materials, regardless of physical form or characteristics, made or received by an agency of the U.S. Government under Federal law or in connection with the transaction of public business and preserved or appropriate for preservation by that agency or its legitimate successor as evidence of the organization, functions, policies, decisions, procedures, operations, or other activities of the Government or because of the informational value of data in them. They allow employees to carry out the mission of an agency, to protect rights, and to ensure accountability. Some records include: Time and attendance records Contracts Original drawings and specifications Project case files Directives Personnel folders As a Government contractor, you come across documents every day that are not Federal records that must also be tracked. These are any U.S. Government-owned informational materials that don't meet the strict terms of the definition discussed earlier. Many of these items are considered business information and may be subject to discovery or Freedom of Information Act (FOIA) requests. Items in the nonrecord category include: Extra copies of documents kept only for convenience or reference, such as copies circulated within an office for information Stocks of forms and publications Catalogs and trade journals Working papers that do not represent significant steps in the preparation of the record copy of official documentation Library or museum materials intended solely for reference or exhibition To maintain good standing, you must follow a range of guidelines for record creation, maintenance, and eventual transfer to the Government. Creation is the production or reproduction of records. It doesn't necessarily mean when the record was originally created; a copy of a record can still be a record. A document can also become a Federal record once it is received by an agency. Records generated outside of an agency can include letters from the public or reports from state and local governments. Maintenance and use is what happens between the time a record is created and the end of its business use. It relates to any activity involving the location, storage, retrieval, handling, and processing of records. For example: Do you need the document to do your job? If so, what do you use it for (research, time tracking, etc.)? How do you access it? Does it need to be included in your agency's file plan? The following tools are available to help you maintain and use records: File plans--written guidelines that describe what to file, how to file it, how to make records more accessible to program staff, and the proper disposition of records when no longer needed for current business Records schedules--specifies the lifespan for all maintained information; will it be kept for years or deleted in a month? File storage--designated locations for your records Assistance-who do you call? Your records officer or records liaison! Once a record has lived a useful and active life, it is considered inactive and moves to either: the retirement phase, in which it is stored by either the Agency or NARA, or directly to the disposition phase At the disposition phase, the record is either destroyed or sent to NARA for permanent preservation, which depends on whether they have been judged to be temporary or permanent. All Federal records must have a records schedule, which is approved by the Archivist of the United States. You cannot destroy anything without first ensuring that it's covered by your records schedule. All of the management techniques and disposition schedules address one key attribute-trustworthiness, which means that the document is what it says it is. You can be reasonably sure it is free from tampering, that it has been handled correctly throughout its life, and that its security was never compromised. The standards of trustworthiness for electronic documents are the same as those for paper-based documents. Trustworthy records, combined with proper records management techniques, are critical to a primary goal of proper recordkeeping--the adequate and proper documentation of Government functions. Period of Performance The period of performance for this contract consists of a base period of twelve months with four, twelve month option periods. Description of Information to include in the Capability Statement : One original and two copies shall be submitted. •1. Detailed capability statement describing your company's experience in providing these types of projects (to include but not limited to electronic surveys, technical consultation for electronic surveys), to other large organizations (list if they were private sector or government). Include in this description an explanation as to how problems are handled, i.e., late projects and whether or not you can handle requests for expedited projects, staff expertise, experience, formal or other training, current in house capability and capacity to perform work described in the statement of work. •2. A list of 3 references, including names of organizations to whom you have provided this type of work for in the past, contract names, telephone numbers, approximate dollar value of the purchase orders or contracts and period of performance for the contract/order. The capability statement should reference Sources Sought Number NIHOD2012321 and shall include the following: (1) Name and Address of the Organization, (2) The size and type of business: Small Business; 8a Small Business; Service Disabled Veteran Owned Small Business; Small Disadvantaged Business, Woman or Women Owned, Alaskan Owned, native Hawaiian Owned, Native American owned and HUBZone as validated via the Central Contractor Registration (CCR). All offerors must be registered in CCR which is located at http://www.ccr.gov/, (3) Point of Contacts with name, title, voice/telephone, facsimile number, and email address, (4) DUNS number, Company Name, Company Address, Current GSA Schedule appropriate to this announcement and all of your applicable business size representations. •3. The intended procurement will be classified under NAICS Code 541512 and the size standard is $25.0 Million. All proprietary information should be marked as such. Statements should also include an indication of current certified business status; this indication should be clearly marked on the first page of your capability statement (preferably placed under the eligible small business concern's name and address). Responses will be reviewed only by NIH personnel and will be held in a confidential manner. All capability Statements sent in response to this SOURCES SOUGHT notice must be submitted to Terita Stevenson, Contract Specialist, 6011 Executive Blvd. Room 539 C, Rockville, MD 20852. All responses must be received by the specified due date and time in order to be considered. ALL INFORMATION SUBMITTED IN RESPONSE TO THIS ANNOUNCEMENT MUST BE RECEIVED ON OR BEFORE THE CLOSING DATE.
