Description
The Department of Veterans Affairs, Syracuse VA Medical Center, Syracuse, NY intends to purchase a USED 135'BOOM LIFT. The government is seeking qualified Service Disabled Veteran Owned Business or other small business categories to determine if a competitive set-aside exists for this opportunity. This is a combined synopsis notice for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Synopsis will be as follows. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55-Corr (2) and DFARS Changes Notice 20120130. DPAS Rating is DO-S10. The North American Industrial Classification System code is 423830 and small business size standard is 100 employees. Required Item and Descriptions are as follows. "Requirements - Boom Lift - USED (NOT Articulating) o135' platform minimum height oTelescopic straight oDiesel Engine o4 Wheel drive oMaintenance free tires oPower in man basket oManufactured 2007 or later oMaximum hours 2,000 oOperates as designed oFOB Destination oWarranty "Include on your quote o1 year service agreement for manufacturer recommended planned maintenance oWarranty Information oProduct Information oProvide pictures sufficient to capture the material condition "Evaluation Criteria - oLowest price technically acceptable "The awardee shall provide: oDemonstration of operational capabilities (as designed) oDocumentation for certification of usage oRepair & maintenance record Submissions received in response to this notice shall be evaluated based on the technical ability and demonstrated past experience to meet the requirement. All those interested should submit, in writing, the company name, address, point of contact, telephone number, and business size; set aside category; a detailed statement regarding technical capabilities and qualifications, and any additional supporting literature. Vendor must provide a DUNS number (http://www.dnb.com/us/) and register with the Central Contractors Registration (Http://www.ccr.gov/) to further qualify for this contract. The provision at FAR 52.212-1, Instructions to Offeror -- Commercial, applies to this acquisition along with additional addenda. A statement regarding the applicability of the provision at FAR 52.212-2, Evaluation -- Commercial Items, is not applicable to this sole source acquisition. Offeror is to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items.
