- Attachment F - HHS Section 508 Priduct Assessment Template Attachment E - SF-LLL 'Disclosure of Anti-Lobbying Activities' Attachment D - Past Performance Survey Attachment C - Certificate of Confidentiality and Non-Disclosure Agreement Attachment B - Privacy Impact Assessment Attachment A -OSHA Respirator Medical Evaluation Questionnaire (29 CFR 01910.134, Appendix C) RFP Attached, is a copy of the Request for Proposal (RFP) for a non-commercial item to provide The Department of Health and Human Services, Federal Occupational Health (FOH) with an Online Respirator Medical Questionnaire and Evaluation Clearance Tool. According to the Department of Labor (DOL) Occupational Safety and Health Administration (OSHA), the primary means to control occupational diseases caused by breathing contaminated air is through the use of feasible engineering controls, such as enclosures, confinement of operations, ventilation, or substitution of less toxic materials. When effective engineering controls are not feasible, or while they are being instituted, appropriate respirators shall be used pursuant to OSHA Respiratory Protection Standard, 29 CFR 1910.134(c)(1)(ii). With the threat of Pandemic Influenza ever present, it is necessary to implement a Mandatory Respirator Protection Program for many Federal agencies. In accordance with the OSHA's Respiratory Protection Standard, cited above, Federal agencies must comply with this requirement to medically evaluate employees before they use a respirator to ensure that they are physically capable of using a respirator without adverse medical effects. This is a total small business set aside. The solicitation number is 12-233-SOL-00109 and it is issued as request for proposal (RFP). Please view attachments. Please read the RFP carefully and in its entirety. The corresponding NAICS code is 518210 and the small business size standard is $25 million. Technical Inquiries and Questions Regarding Solicitation All questions regarding the solicitation, of a contractual nature or technical nature, must be submitted in writing to Shayla Gross, Contract Specialist, via email at Shayla.Gross@psc.hhs.gov in accordance with the instructions found under Section L of the RFP by 12:00 PM Eastern Standard Time March 12, 2012. PSC will not consider any questions received after that date. Quote Submission Offerors who intend to submit proposals in response to this solicitation must respond accordance with the Section L.7 PROPOSAL SUBMISSION in the RFP. All offers must arrive NO LATER THAN 12:00 PM EASTERN STANDARD TIME MARCH 26, 2012. Late submissions, modifications, and withdrawals of bids will be processed in accordance with FAR 52.215-1.
