Description
The VA Healthcare Network Upstate New York (VISN 2) desires to enter into a multiple award contract with Joint Commission (JC) Certified Contractors to provide storage, delivery, set-up, repair, instruction-in-use, and/or pick-up of Government purchased items, in accordance with all terms, conditions, provisions, specifications and schedules herein. Deliveries and/or pick-ups shall be made at such times and in such quantities as specified by the VA Medical Center Contracting Officer or his/her designated representative (COR). Joint Commission Standards for the Accreditation of Home Care, Section 1, Chapters I through VII, and Section 2, Chapters VIII through IX are applicable to service provided under this contract. The VA intends to award approximately two (2) to five (5) Firm Fixed-Price, Indefinite Delivery, Indefinite Quantity contracts pursuant to this solicitation for commercial items, however the VA does reserve the right to award to one contractor, if it is determined to be in the best interest of the Government. The VA shall determine (based on certification, training, past performance, and price proposals) the individual proposals judged to be most advantageous to the Government. Offerors should submit a price for each item for each major site that they are interested in providing services to. Proposals may be submitted for one or all of the major sites. This is a Service Disabled Veteran Owned Small Business Set-Aside Cascaded Procurement. ALL responsible small business concerns are encouraged to submit proposals. VA intends to evaluate offers and make award as indicated. Service Disabled Veteran Owned Small Business Participation - In accordance with VAAR 819.7005, any award for the storage and delivery of durable medical equipment from this solicitation, will be made on a competitive basis to eligible, verified, service disabled veteran owned small business (SDVOSB) concerns, provided that a minimum of two (2) competitive offers (technical and cost) are received from such concerns. If a minimum of two (2) offers from eligible SDVOSB are not received, the award for the services/supplies specified within the solicitation will be made to a veteran-owned small business (VOSB) in accordance with VAAR 819.7006, provided that a minimum of two (2) competitive (technical and cost) offers are received from qualified small business concerns. If a minimum of two (2) offers from qualified VOSB are not received, award for services/supplies specified within the solicitation will be made to a qualified 8(a), HUBZone, or woman-owned small business in accordance with FAR 19.8 and 19.13. These three programs have equal parity according to SBA. If award cannot be made to a qualified 8(a), HUBZone or woman-owned small business, the award will be made to a small business in accordance with FAR 19.5, provided that a minimum of two (2) competitive (technical and cost) offers are received from qualified small business concerns. The proposed contract period shall be for a base period, April 5, 2012 (or date of contract award) through April 4, 2013, with four (4) on-year option periods. The applicable North American Industry Classification System (NAICS) Code is 621610 - Home Health Care Services, and the size standard is $13.5 million. The solicitation will be available on or about March 12, 2012. Offerors who have the ability to provide these services shall submit a WRITTEN REQUEST ONLY for the solicitation package.
