- "This request for sources sought is for informational and market research purposes only. All information and action provided is completely on a voluntary basis only. Nothing stated in this sources sought by any Government Official shall be treated as direction or request to the vendor for action. All direction or requests will come from the Contracting Officer; as only they have the legal authority to bind the U.S. Government. STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Extension of Smart Home Technology to Individual Homes 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. a.Provide one full-time software engineer/developer resident at the Tampa VA for 1 year. b.Provide one full-time biomedical/biomechanical engineer resident at the Tampa VA for 1 ear. c.Provide one-year consultation for a computer scientist with expertise in machine-human collaboration using natural language processing. d.Professional installation-service including all necessary parts, components, travel, management as required. e.Provide for one-year, any necessary replacement parts, equipment and consumables (ie. batteries, cables). f.Provide consulting support and service for in house design of sensors components integration. Ubisense is the manufacturer and installer of the real time location system (RTLS) and the developer of the custom software used in the Tampa VA Smart Home. For this project the contractor will provide all personnel equipment and installation services needed to equip the 5 veteran homes with our Smart Home technology. This includes a 2-year hardware and software maintenance plan as well as a redesigned, non-industrial sensor housing suitable for long-term use in a home environment. 3. Background: The Tampa VA Smart Home has been fully integrated into the Tampa Assistive Technology (AT) program providing cognitive assistive technologies to Polytrauma TBI and other cognitively impaired veterans. The original Tampa Smart Home was designed and developed in partnership with Tampa VA staff by Ubisense, Inc. as part of a contract in 2010. Specifically, Ubisense was contracted to integrate their ultra-wide band tracking hardware with the Smart Home artificial intelligence software (which Ubisense also developed). 4. Performance Period: To install the Smart home into 5 veterans home within 1 year (365 days) from the day of the award of the service contract with an option to extend the service contract for another 2 years (3 years total) provided that funds are available for years two and three. There may be circumstances that delay the installation of the Smart Homes and cause the project to extend beyond 365 days, such as subject recruiting schedules or other extenuating factors (for example access into veterans homes). The contractor will provide the personnel and services and complete the installation of the 5 Smart Homes at no additional cost during this extended contract period. The contractor will also provide for a warranty period of 1 year following the end of the contract period (or extended contract period if an extended period is required), to rectify emerging software and/or hardware failures. The contractor will not be obligated to add any additional functionality during the warranty period (unless the contract is renewed). There may be also circumstances where the government determines that it is not possible to install 5 Smart Homes (for example, because of recruitment or unforeseen technical difficulties). Should this become evident the Government will, in writing, notify the contractor that term of the contract will be modified such that that the number of installations will be reduced. If this should occur any remaining funds allocated to the excised home(s) can be rolled into extending the term of such personnel as the programmer, consultant and biomechanical engineer, beyond their 365-day contract term. How the funds will be reallocated among personnel shall be subject to negotiation between both parties (the Government and the contractor). The sites of the Tampa homes are anticipated to be within a reasonable distance from the Tampa VA (within the greater Tampa Bay Area). The contractor will make available consulting services, supply materials and equipment as necessary in order to extend and maintain the Smart Home into 5 individual homes. If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail and full payment will be provided to the contractor, however the specified personnel will remain on-site at the Tampa VA for the duration of the 365 days. 5. Type of Contract: a. Service, installation and design: firm-fixed-price 2012 b. Personnel: firm-fixed-price 2012 B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Contractor will provide the services: a) a one full-time software engineer/developer that will be resident at the Tampa VA for the duration of the contract; b) one full biomedical-biomechanical engineer, and d) one part-time computer scientist consultant to assist in developing a proof of concept virtual rehabilitation companion ( buddy ) based on natural language processing. The specific development, changes and addition of features shall be determined in consultation with Polytrauma Transitional Rehabilitation Program (PTRP) and AT staff. Payment will be made after each of the following deliverables is verified to be in good working order to provide the funds necessary to complete the next deliverable. The design of upgrades and additional features that will benefit cognitively impaired veterans will be determined prior to installation by the design team which will include the above mentioned contractor personnel and select members of the Tampa COE (Center of Excellence), PTRP (Polytrauma Transitional Rehabilitation Program), and PNS (Polytrauma Network Service) staff. Specific milestones/deliverables are listed below; the order of completion will be negotiated between the Tampa Team and contractor. Deliverable 1: Delivery of sensors, Smart Home components, and all hardware, and other items ready for installation. Deliverable 2: Hire consultant to start the design of the proof of concept natural language processing virtual companion ( buddy ). Proof of concept should be completed within the 365 of award of contract. Deliverable 3: Hiring of above-mentioned personnel as soon as practicable after the award of the contract. The personnel shall work in conjunction with PTRP and PNS clinical staff to identify, plan, develop and modify features in the Smart Home for each veteran s home (5 in total). Deliverable 4: Installation of equipment and commissioning and testing of Smart Homes. The installation can occur one at a time or in parallel if appropriate. Deliverable 5: Design of prototypes sensor housing using a professional design service. Deliverable 6: Design, manufacture and delivery of sensor housing after written acceptance by the Tampa VA team before installation of the sensors in homes. E. SCHEDULE FOR DELIVERABLES 1. The contractor shall complete the delivery of all items and installations as specified in Section A and Section D. The contractor will be required to provide each item, service, or personnel as specified in the table below. 2. If for any reason any deliverable cannot be delivered within the required time frame, the contractor is required to explain why in writing to the CO and Tampa project team, including a firm commitment of when the work shall be completed. This notice to the CO and Tampa project team shall cite the reasons for the delay, the impact on the overall project, and a statement of how such delays will be avoided for future deliverables. The CO will then review the facts and issue a response, in accordance with applicable regulations. 3. Order of deliverables is flexible. DeliverableEst. Date of Completion Deliverable 1: Sensors (50) + component parts Within 30 days of contract award or ASAP if supplies are backordered. Deliverable 2: Consultant services Within 30 days of contact award Deliverable 3: Hiring of Personnel 1 month after award of contract. Extension may be requested if acceptable candidates cannot be found within 1 month. Deliverable 4: Smart Home installation per house (maximum of 5) Includes house site survey. As needed within the initial 12-month contract period or negotiated.
