Description
Request for quotation - Form SF18 Statement of work for fiber optics installation and terminiation effort. COMBINED SYNOPSIS/SOLICITATION Fiber Optics Installation and Termination Services (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: RA-133R-RQ-12-0089MH (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55. (IV) This solicitation is being issued as Total Small Business Set-Aside. The associated NAICS code is 238210. (V) This combined solicitation/synopsis is for purchase of the following commercial items: In order to bid, the Offeror must be Corning/Siecor certified. Reference the Statement of Work for detailed requirements. CLIN / DESCRIPTION / QUANTITY / UNIT OF MEASURE 0001 / 48 strands of single mode fiber optic cable, provided by the contractor, shall be run between the DSRC MDF room 2B601 and NIST Building One room 2001 including inner duct / 1600.00 / FT 0002 / CORNING Closet Connector Housings Model CCH-04U to be installed at both the DSRC MDF 2B601 and the NIST Building one room 2001 / 2.00 / EA 0003 / CORNING CJP-02U Cable Management System will be installed / 2.00 / EA 0004 / Terminate 24 (12 pairs) of the 48 strands and test / 24.00 / EA 0005 / Optionally, terminate the remaining 24 (12 pairs) of the 48 strands budget depending and test / 24.00 / EA (VI) See attached statement of work, and Department of Labor Wage Rates: WD 05-2081, Revision No. 11, dated 06/17/2011 which can be found on www.wdol.gov. (VII) Date and places of performance and acceptance shall be at National Oceanic and Atmospheric Administration (NOAA) David Skaggs Research Center (DSRC) Building and the National Institute of Standards and Technologies (NIST) Building One located at 325 Broadway, Boulder, CO 80305. Delivery shall be FOB Destination, Boulder, CO. Period of performance shall be completed by April 30, 2012. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (FEB 2012), applies to this acquisition. The offerer will submit a quote for the items in Section (V). Inquiries: Offerors must submit all questions concerning this solicitation in writing to Marilyn Herrera at Marilyn.Herrera@noaa.gov. Questions should be received no later than 12:00pm MST on February 29, 2012. All inquiries will be sent to the technical contact for response. Any responses to questions will be made in writing, without identification of the questioner, and will be included in an amendment to the solicitation via FedBizOpps. Even if provided in other form, only the question responses included in the amendment to the solicitation will govern performance of the contract. (IX) Site Visit - A mandatory one-time site visit will be held on February 22, 2012 from 1:00 PM - 3:00 PM MST. You are all advised to arrive 30-40 minutes prior to the start of the meeting. With the meeting being held on a Federal Campus you will have to stop at the Visitor center and are subject to search. Please bring a Federal or State issued ID (with picture). Inform the guards that you are attending the meeting. Someone from the team will escort you to the meeting location. It is suggested that all attendees bring a copy of the RFQ with them. The government will attempt to answer simple technical or factual questions during the site visit, as a convenience to the offerors. No reliance may be made upon such answers. Only questions submitted in writing to Marilyn Herrera (by email, Marilyn.Herrera@noaa.gov) and answered in writing by the government may be relied upon for purposes of contractual interpretation. (X) FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), applies to this acquisition. Best Value to the Government. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technical Knowledge: Offeror's quote shall include resumes of personnel performing work on this project. 2) Past Performance: Offeror's quote shall include at least two references including the point of contact name, phone number, full address and email address. 3) Price. (End of Provision) (XI) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications (FEB 2012) - Commercial Items with its quote. The offeror shall complete the annual representations and certifications electronically at http://orca.bpn.gov. In the event the contractor makes any changes at the direction of any person other than the Contracting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract terms and conditions, including price. The COR may be changed at any time by the Government without prior notice to the contractor by a unilateral modification to the contract. The COR is also responsible for the final inspection and acceptance of all deliverables and such other responsibilities as may be specified in the contract. (2) The COR is not authorized to make any commitments or otherwise obligate the Government or authorize any changes which affect the contract price, terms or conditions. Any contractor request for changes shall be referred to the Contracting Officer directly or through the COR. No such changes shall be made without the express written prior authorization of the Contracting Officer. The Contracting Officer may designate assistant or alternate COR(s) to act for the COR by naming such assistant/alternate(s) in writing and transmitting a copy of such designation to the contractor. (End of clause) (XV) Defense Priorities and Allocations System (DPAS) and assigned rating do not apply.
