SOURCES SOUGHT SYNOPSIS REQUEST FOR INFORMATION ONLY THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. THE VA IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. In accordance with FAR 15.201(e), under Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. The Department of Veterans Affairs (VA) is seeking potential sources capable of providing Orthopedic services as well as diabetic and customized footwear to be delivered to beneficiaries of the VA Butler Healthcare System. Synopsis: 1. VA Butler Healthcare is seeking sources to provide orthotic appliances and related services to veteran beneficiaries of the VA Butler Healthcare system. Contractor shall provide discount to L-code prices (L3200 through L3500) applicable to established prices in the geographical area published by the Healthcare Financing Administration's (HCFA) the Healthcare Common Procedure Coding System (HCPCS). Contractor shall provide service including evaluation, manufacture, fitting and adjustment of Orthotic devices by its qualified Orthotist. Veterans who are eligible for orthotic services may be provided with arch supports, shoe modifications, functional foot orthotics, depth inlay shoes, healing shoes, custom-made orthopedic shoes, and alternative footwear. Footwear and modifications must be prescribed by the appropriate VA clinician. The majority of devices is purchased from the Contractor and is inclusive of all fittings and adjustments. The-Contractor will bill VA for measuring and fitting any VA-furnished devices utilizing L-4205 Services will be provided to eligible veteran patients on an inpatient and outpatient basis, upon referral of a VA Butler credentialed physician or by a practitioner working under the direct supervision of a physician (i.e. clinical nurse specialist, physician's assistant, and nurse practitioner). All services provided the Contractor shall be furnished in accordance with a plan of care established by the physician responsible for the patient's care and may not be altered in type, amount, frequency or duration, except in cases of an adverse reaction to a specific service or treatment, upon written instructions from the physician. The Contractor will provide on-site services to the government by qualified Orthotists at VA Butler Healthcare, and associated community based outpatient clinics (CBOC) at the same time as podiatry clinic. Currently podiatry meets at VA Butler on Mondays 8am-12noon and Thursdays 8am-4pm. Podiatry is at the Mercer CBOC the 4th Tuesday of the month 8am-4pm and at the New Castle clinic the 1st and 3rd Tuesday 8am-12noon. The Contractor will be notified 24 hours in advance of whether clinics are canceled on any given day. Patient walk-ins are prioritized by medical emergency and seen or scheduled as necessary throughout the day. 2. The place of performance includes the following counties in Pennsylvania: Allegheny, Armstrong, Beaver, Clarion, Lawrence, Mercer, and Venango; Ohio Mahoning 3. The NAICS Code associated with this request for information is: 339113 4. Responses to this RFI should be emailed to andrew.taylor3@va.gov not later than March 14, 2012 and should include the following information: a. Name and address of company; b. Capability statement; may include JACHO Accreditation. If not JCAHO accredited, the contractor will have knowledge of and be in compliance with JCAHO standards and must be prepared to provide evidence that it is compliant overall with the JCAHO standards as described in the Comprehensive Accreditation Manual for Home Care re Home Medical Equipment and Clinical Respiratory Services. If accreditation is lost during the contract period, the contractor will immediately notify the Contracting Officer (CO). Subcontractors must follow JCAHO standards and will be monitored by the contractor for compliance. c. Business size; d. GSA/FSS Schedule number. (if available) e. Identification of any socioeconomic categories (SDVOSB, VOSB, 8a, Hub, WOSB, SB) the company falls under. f. Any other pertinent information. g. A letter or memorandum indicating a positive intention to provide a proposal if requirement is solicited. The letter should be signed on behalf of your company by a senior procurement official, senior management, owner, or partner (an individual with authority to bind the firm). PLEASE LIMIT RESPONSE TO 10 PAGES 7. Response date (Deadline): Responses must be received no later than 4:00 p.m. on Monday March 14, 2012. Responses received after March 14, 2012 may be included with our research but any future consideration for this procurement is not guaranteed if the response is received after the deadline. 8. Veterans First Contracting Program In accordance with IL 049-07-08 "Veterans First Program" the VA, will place SDVOSBs and VOSBs first and second, respectively, in satisfying VA's acquisition requirements when possible. Certain conditions must be met. All VOSBs, including SDVOSBs, must register in the VetBiz.gov Vendor Information Pages (VIP) database, available at www.VetBiz.gov, and CVE certified to be eligible for award of a contract. 9.
