- Wage Determination, Rev. 10, 2005-2133, 13 June 2011 - Attachment 3 Maps - Attachment 2 Performance Work Statement, 23 January 2012 - Attachment 1 Solicitation - FA6703-12-R-0001 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested for this solicitation. The solicitation number is FA6703-12-R-0001 and is being issued Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55. The NAICS code for this requirement is 562111 with a Small Business Size Standard of $12,500,000.00. This requirement is a total set-aside for small businesses. Dobbins Air Reserve Base (DARB) (Marietta), GA is soliciting for contractors to provide Refuse Pickup, Removal, and Disposal Services. The contract contains four (4) contract line item numbers (CLIN): CLIN 0001: Municipal Solid Waste, twelve (12) Months; CLIN 0002: Unscheduled Pick-ups, one (1) LS; CLIN 0003: Construction & Demolition Debris, one (1) LS; CLIN 0004: Landfill Tipping Fees, one (1) LS. The contract performance period is one (1) basic year with four (4) 12 month option periods. A Performance Work Statement dated, 23 January 2012, is included with this Request for Proposal. Work includes but is not limited to: providing dumpsters/containers of various sizes throughout the base, (containers must be painted to match the color scheme of the base). Frequency is for pickup twice weekly on Tuesday and Friday and disposed at a permitted off-base disposal site. The anticipated contract is Firm-Fixed Price for all work with the exception of dump fees which will be reimbursed upon submission of certified invoicing. All offerors must be registered in the Central Contractor Registration (CCR) database prior to submitting a proposal to be considered for award. The following provisions and clauses apply to this acquisition: 52.212-1, Instructions to Offerors – Commercial Items; 52.212-2, Evaluation—Commercial Items; 52.212-3, Offeror Representations and Certifications – Commercial Items; 52.212-4, Contract Terms and Conditions – Commercial Items. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders—Commercial Items applies to this acquisition. Request for Proposal will be issued on or about 3 February, 2012 with a closing date of 5 March 2012. All responsible parties may submit an electronic proposal via e-mail which shall be considered by the agency.
