- SF 1449 (Solicitation) with Clauses Statement of Work IAW Title 29 C.F.R. Part 4, Section 4.6(d), the contractor is required to pay no This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA4855-12-Q-0035 and is a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-55, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20111220, and Air Force Acquisition Circular (AFAC) 2012-0104. The North American Industry Classification System (NAICS) is 711510 (Musician Independent Artists). Small Business Size Standard is 7.0 M. The Offeror must be an authorized, qualified, and certified vendor and must quote on an all or none basis. Written quotes are required (oral offers will not be accepted). Only one contract will be awarded. Line Item: 0001 - Chapel Catholic Musician, Total 70 hours per year Service Non Personal: Contractor to provide Music for the Catholic Services for Cannon Air Force Base Chapel, Clovis NM in strict accordance to Statement of Work (SOW). Period of Performance 1 March 2012 through 30 September 2012 to include 4 option years, pricing is hourly base. 0002 - Option Year 1 Services for period of performance 1 October 2012 through 30 September 2013, 120 hours/year. 0003 - Option Year 2 Services for period of performance 1 October 2013 through 30 September 2014, 120 hours/year. 0004 - Option Year 3 Services for period of performance 1 October 2014 through 20 September 2015, 120 hours/year. 0005 - Option Year 4 Services for period of performance 1 October 2015 through 30 September 2016, 120 hours/year. RESPONSE TIME: Request for Quotation will be accepted at the 27th Contracting Squadron, 110 Sextant Ave. Bldg. 600, Cannon AFB NM 88101-5214 NO LATER THAN 2:00 pm Mountain Time on 28 February 2012.
