Description

DESCRIPTION/SPECIFICATIONS/WORK STATEMENT 1. GENERAL: a. Background: The Department of Veterans Affairs Medical Center, 13000 Bruce B. Downs Blvd., Tampa, FL has a requirement for Durable Medical Equipment (DME) service for the veterans of James A Haley Veterans' Hospital, Tampa, FL and VA Pasco OPC, 9912 Little Road, New Port Richey, FL 34654, Lakeland CBOC, 3240 S. Florida Avenue, Lakeland, FL 33803, Brooksville CBOC, 14540 Cortez Blvd., Suite 200 Brooksville, FL 34613, and Zephyrhills CBOC, 6937 Medical View Lane, Zephyrhills, FL 33541. b. Scope of Work: The Contractor shall provide office and storage space for VA owned equipment (6000 contiguous square feet) and includes phone, fax, and internet service. The Contractor shall provide delivery and pick-up, receiving, storage, set-up, assembly; installation, instructions, sanitizing, repair and maintenance of VA owned DME to eligible VA beneficiaries as required by the VAMC Tampa, PASCO OPC and CBOC's. Services provided shall be all inclusive of deliveries and transportation to include set-up/delivery of equipment, safety inspections, and in-service training for beneficiaries to include patient education. c. Contractor requirements: 1. Contractor shall have a physical location which houses the service office, warehouse, dispatch of vehicles and other functions related to the performance of services in Central, FL for the duration of the contract. The area of service is the entire state of Florida, with a few exceptions to extend 0-30 miles beyond the state borders. 2. Contractor shall have minimum of (5) vehicles for transport of durable medical equipment. 3. Contractor shall have a minimum of 5 drivers, 2 technicians and 4 Administrative support staff for receiving phone calls, scheduling, deliveries, servicing of equipment and performing administrative functions. 4. Contractor shall be required to comply with all specifications and to adhere to the standards of care as designated within this contract and in compliance with The Joint Commission, to include: delivery, set-up, pick-up, cleaning, storing, education, patient home safety, inventory of DME as required to meet the standards described in the Equipment Management Chapter of the Home Health Care standards prescribed by The Joint Commission. 5. Contractor shall be required to comply with Joint Commission standard for conducting maintenance of specialty wound care mattresses. 6. Contractor shall be licensed as a Home Medical Equipment Provider in the State of Florida. 7. If the bidder maintains branches and/or agents through whom business may be transacted, they must submit a list thereof with the bid. If awarded a contract, they must transmit all information concerning the agreement between such branches and/or agents authorizing and instructing them to furnish supplies and/or services covered. 8. HSS: No contract shall be awarded nor payment made to any individual or entity that appears on the Health and Human Services Office of Inspector General's (HSS/OIG) List of Excluded Individuals/Entities. 9. HIPAA Compliance -The successful Contractor shall be required to be in compliance with Health Insurance Portability and Accountability Act of 1996 (HIPAA) requirements.
