- Statement of Work Job Site: Washington DC VA Medical Center 50 Irving Street NW Washington, DC 20422 Job Description:3rd Floor East Restrooms. ( Public, Patient, Staff and HAC Closets ) CLC Patient Restrooms and Showers ( Total 50 Rooms. CLC Tub Rooms ( Total 2 Rooms ) Basement Level Mens and Womens Restrooms next to Canteen. Total Sq. Ft9762 Scope of Work:1. Remove existing failed glaze coatings from tile and grout. 2. Apply Permatect Grout Topping in all areas. 3. Apply Permatect Microguard Inorganic Protective Coating to all tile and grout in all areas. Vendor Requirements. 1.Vendor must have Restoration Services with a Permanent Protective Inorganic Protective Barrier on GSA Federal Supply Contract. 2.Vendor must be Certified Permatect Microguard Applicator Vendor Responsibilities: 1.Vendor to provide all chemicals, equipment, supplies and personnel to complete the above process. 2. Vendor is to coordinate work schedules with stipulated facility contact. 3. Vendor is to sign in at facility (when required) and acquire the proper ID badges prior to beginning work each day/night and sign out at the end of shift. If back ground checks are required that will take more than 1 hour, vendor will pre-submit all required personal information to allow pre-processing to occur. 4.Review job area with the stipulated facility contact. Make sure that the area matches the area defined in the purchase order and work schedule is reviewed and concurred by the stipulated facility contact. 5.Secure the area to be prepared. Install a protective floor mat in front of doors ways. If extension cords are crossing walking areas, secure with tape. Be mindful of the guest in the facilities. Keep all equipment in a safe and discrete location. 6. Areas and square footage for the grout cleaning. TWE in the following areas to be clean Area #1 Sq. Ft. in the CLC tub rooms 2H211 & 2K211 and restroom GE206 & GE207 next to Cafeteria- 1,448 Area# 2 Sq. Ft. in the 3rd Floor public restroom - 3,464 Area# 3 Sq. Ft. in CLC Patient for 50 restroom - 4,850 Total 9,762 Sq. Ft. 7.Demobilization a.Walk work area and certify all tools and trash are removed from site. b. Properly remove all equipment. c. Review any necessary actions needed by facility in reference to additional precautions and/or actions necessary to open the area. d.Sign out and review work completed with stipulated facility contact.
