Description
DESCRIPTION/SPECIFICATIONS/WORK STATEMENT 1.GENERAL: The Contractor shall provide all supplies, materials, minor parts lower than $25.00,all equipment, labor, tools, supervision, management and transportation to perform all tasks identified below, from February 1, 2012 to September 30, 2012, for all Hoods the Miami VA, Radiology and Broward Clinics All equipment shall be maintained in proper operating condition as specified by the manufacturer. Vendor is to have proven access to original equipment manufacturer's (OEM) parts, manuals and schematics, which must be available on site to perform the service. Vendors must provide their own OEM manuals and schematics, which must be available on site at the time of service. All work is to be performed in accordance with the guidelines established by Federal, State and local ordinances, with Manufacturer's Manual and Quality Control Manual, and with all terms, conditions, provisions, schedules and specifications provided herein. Contracting Officers Technical Representatives (COTRs) for this agreements, and Their VA phone numbers are: Robert A. Pastorello, Chief, Biomedical Engineering Service (305) 575-3183 Lydia Parets, Lead Biomedical Technician (305) 324-4455 x4466 2.PREVENTIVE MAINTENANCE (PM)/CERTIFICATION: This contract is for the semi-annual certification of all Hoods used in the Pathology Labs and throughout the Miami VA Healthcare System. Contractor (or representative), after checking through security at the main hospital entrance, shall sign in at the Biomedical Engineering Service Office, B116 of the main hospital building, telephone (305) 324-4455 extension 3655. After completion of PM or emergency service contractor shall notify users of completion, users will verify completion of work and sign service ticket. A copy of the service ticket will be returned to the Biomedical Engineering Service Office. Failure to follow these procedures may result in delay of or nonpayment. PM inspection(s)/Certification will be performed semi-annually during the month(s) of January and August at a time mutually agreed in advance, between the Contractor, Biomedical Engineering and the Contracting Officer's Technical Representative (COTR). PM(s) will be performed exclusive of emergency service during regular administrative working hours unless specifically authorized by the Contracting Officer. The Contractor will be required to furnish a complete checklist of items that are to be accomplished under the preventative maintenance/certification service. PM /Certification will follow the manufacturer's specifications and include electrical safety testing (when applicable), lubrication, adjustments, calibration, testing and replacement of faulty parts (within scope of this contract) and parts which are likely to fail at no additional charge. The equipment will be returned to the operating condition specified by the manufacturer. 3.EMERGENCY REPAIR: The Contractor shall provide emergency repair service within 24 hours after notification by the Contracting Officer or the COTR during regular administrative working hours. Contractor will report to Biomedical Engineering upon arrival. If emergency repair is requested and approved by Contracting Officer or COTR for after normal administrative hours, Contractor must report to the VA Police on arrival. The Contractor shall provide a twenty-four hour emergency telephone number for repair service. TOLL FREE NUMBER: 952-253-4512 4.REPAIR PERSONNEL: Personnel performing maintenance and repair must be fully qualified, competent Field Service Engineers (FSE). "Fully Qualified" is based upon training and on experience in the field. The Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSE's for each make and model the Contractor services at the VAMC upon request by the Contracting Officer. The Contracting Officer and/or the COTR specifically reserves the right to reject any of the Contractor's personnel and refuse them permission to work on the VAMC equipment. If subcontractors are used they must be approved by the Contracting Officer prior to contract signing. The Contractor shall submit any proposed change in subcontractor(s) to the Contracting Officer for approval/disapproval at least three (3) months in advanced. 5.PARTS: This contract includes all minor parts under $25.00 if required to certify hoods a. Parts excluded from this contract are: All parts are excluded other than parts under$25.00 b.Excluded Services: Contractors will not be obligated to provide services under this contract for damage to, or destruction of, equipment covered where damage or destruction is: i) the result of, or caused by, fire or explosion of any origin as a result of riot, civil commotion, aircraft, war or any act of terrorism, or any act of nature including, but not limited to, lightning, windstorm, hail, flood or earthquake. ii) caused by VAMC misuse or abuse of such equipment by contamination due to spillage, interpretation of data, installation of unauthorized field modifications or accessories not listed or included in this contract. The Contractor must give reasons for excluding services and/or extra charges due to these problems in writing to the VA within three (3) days of completion of work. Failure to provide this information will nullify such charges and the Contractor will be responsible for them. Further, no equipment shall be removed from VA owned equipment due to these conditions without the written consent of the Contracting Officer. If the contractor maintains that the problem is with reagents, calibrators, operator error or controls, the burden of proof shall be on the Contractor; however, if the equipment is found to be at fault, the Contractor shall pay for all tests or services sent out by the VA while equipment is inactive during the dispute period. 6.SYSTEM/SOFTWARE UPGRADES: NA Contractor will be responsible for enacting service recalls/hazard alerts published by the manufacturer or reporting body at no extra cost. 7.LOANER EQUIPMENT: NA 8.RESPONSE TIME: Contractor will commence on-site performance for unlimited emergency service within 24-hours due to equipment malfunction, Monday through Friday, excluding Federal holidays, between the regular administrative working hours of 8:00 a.m. to 5:00 p.m. Contractor will have equipment functioning to manufacturer's specifications within 72 hours of original call from VA. 9. HOURS OF SERVICE: Normal hours of coverage are Monday through Friday from 8:00 a.m. to 5:00 p.m., excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by the COTR. Federal holidays observed by the VAMC are: Labor DayColumbus Day Martin Luther King DayVeterans Day President's Day Memorial Day Independence Day 10.REPORTS: Upon completion of the emergency repair, the Contractor shall submit to the Biomedical Engineering Section a service report indicating what repairs, service, calibration, etc. were performed. Each Service Ticket Report must, at a minimum, document the following data legibly and in complete detail: A.Name of Contractor B.Name of FSE who performed services. C.Contractor Service ESR Number/Log Number. D.Date, Time (starting and ending), equipment downtime and hours-on-site for service call. E.VA PO# (s) covering the call, if outside normal working hours. F.Description of problem reported by COTR/User. G.Identification of equipment serviced: inv. ID#, manufacturer's name, device name, model #, serial #, and any other manufacturer's identification #s. H.Itemized description of service performed including: labor and travel, parts (with part #s) and materials and circuit location of problem/corrective action. I.Signatures: 1.FSE performing services described. 2.VA employee who witnessed service described. This report shall be signed by the using service designee attesting that the equipment is in satisfactory working order and is functioning at the acceptable level of performance. A copy can be made at the Biomedical Engineering Office (B116) when the contractor signs out. NOTE: Payment will be withheld pending resolution for any of the following: (1)No service report was submitted. (2)A service report was submitted but was not legible written. (3)A service report was not signed by the proper VA representative, or (4)A service report was not presented to or made available to Biomedical Engineering at the time of the service. 11.RESPONSIBILITY OF THE CONTRACTOR: The Contractor must obtain authorization from the Contracting Officer or the COTR Designee for any repairs required outside the normal administrative working hours and will be invoiced separately. Separate travel charges will not be accepted for overtime work that is begun during regular hours. The approval of the Contracting Officer must be obtained before removing equipment to the Contractor's plant. No transportation charges will be allowed for either repairmen or equipment to or from the Contractor's plant. The Contractor will be responsible for damage or loss of the equipment. The Contractor is responsible for providing personal protective equipment (PPE) for their employees, and all standard safety practices must be in place and followed by their employees. Failure to follow such safety practices could result in suspension of work by VA Safety Authorities. 12.INDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VAMC. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police & Security Service. The VAMC will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any buildings at the VAMC. Possession of weapons is prohibited. Cellular telephones and/or two-way radios are prohibited from use within critical care areas of the medical center. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.
