- Attach 3 - Abbreviations Attach 2 - Proposed Staffing Sheet Attach 1 - Past Performance Performance Based Statement of Work THIS PROCUREMENT IS ISSUED ON AN UNRESTRICTED BASIS. The Contractor shall provide Teleradiology Services in support of the Sells Indian Hospital and its service unit clinics. This procurement is conducted pursuant to the authority of Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)); FAR 37.4, Nonpersonal Health Care Services Contract (10 U.S.C. 2304 and 41 U.S.C. 253); and FAR 37.6, Performance-Based Acquisition. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-54. The associated North American Industry Classification System code is 621512 with a size standard of $13.5 million. For the calculation of annual receipts and number of employees see parts 121.104 and 121.106 in SBA's Small Business Size Regulations, http://www.sba.gov/regulations/121/ The Tucson Indian Health Service has a requirement for Tele-Radiology Services to provide off-site radiological interpretations/consultations to its medical facility staff 24 hours a day / 7 days a week. The contractor shall ensure that all studies are read and transcribed into the Tucson Area Indian Health Services systems within the time frames specified within the attached Performance Based Work Statement. The Tucson Area Indian Health Service utilizes Digital Imaging and Communications in Medicine (DICOM) protocol and Thin-Air Teleris software. The estimated volume of work load for diagnostic films range from 9,000 to 12,000 annually. The contractor shall determine the number of radiologists required to perform the services in order to meet the standards for this requirement. The IHS intends to award a performance-based Indefinite Delivery - Indefinite Quantity (IDIQ) contract for a base year with four 12-month options. Award will be based on Best Value to the Government. Written quotes are required (oral offers will not be accepted). To ensure best possible consideration interested offerors must ensure their proposal submissions fully comply with the OFFERORS INSTRUCTIONS outlined under page L-3 in the attached solicitation package. Incomplete packages will not be accepted.
