Description

The Department of Veterans Affairs, VA Montana Health Care System, Ft. Harrison MT, is issuing this combined synopsis/solicitation for a commercial item prepared in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. THE ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is issued as a Request for Quote.. This is Full and Open under the North American Industrial Classification Standard code 339112. Delivery location will be VA Medical Center, Salt Lake City, Utah. The Rocky Mountain Acquisition Center intends to negotiate a firm-fixed price contract for the acquisition of the item listed below. It is the government's intention to award sole source to Terumo Medical Corporation. All responsible sources may submit a quote which shall be considered by the agency. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55. Item 1QuantityUnitPrice 9005081 Fresenius Continuous Autotransfusion 3EA System Plus (C.A.T.S) Item 2QuantityUnitPrice 9108441 ATH Collection Reservoir Holder 3EA Evaluation of responses will be conducted and evaluated under the provisions of FAR Part 12, Commercial Items, and FAR 13.5 Simplified Acquisition Procedures. The responsible offeror must be registered in the Central Contractor Registration (CCR) database www.ccr.gov to be considered for award. Offeror must also be registered in the Online Representations and Certifications (ORCA) database http://orca.bpn.gov system per FAR 52.212-3, or submit a completed copy of the Offeror Representations and Certifications - Commercial Items with their quote. All responsible sources may submit a quote in response to this solicitation, which shall be considered. The Rocky Mountain Acquisition Center intends to award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government (best value). The Government may reject any quote that is evaluated to be unrealistic in terms of program commitments, including contract terms and conditions, or unrealistically high or low in cost/price when compared to Government estimates, such that the quote is deemed to reflect an inherent lack of competence or failure to comprehend the complexity and risks of the program. Only new equipment with warranty will be accepted.
