- Bid Schedule Combine Synopsis Statement Of Work (SOW) (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is F2N02A1307A001; this solicitation is a Request for Quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-48. (iv) There is no small business set-aside for this acquisition. This acquisition will be full and open competition; the North American Industry Classification System (NAICS) code is 611710. The business size standard is $7million the following commercial items are requested in this solicitation: (v) CLIN: 0001- BASE YEAR, PERIOD OF PERFORMANCE: 1 JAN 2011 - 31 DEC 2012. DESCRIPTION OF SERVICES: Non-personal services in support of U.S. Air Force chapel services. The Catholic Pastoral Associate is responsible for the management of the Catholic program under the guidance of the Chaplain in charge of the Catholic Community. The duties include planning, management, training, administration and coordination of activities, programs, schedules, liturgies and agendum pertaining to the Lajes Field Catholic Community (Holy Trinity Parish) as specified in the PWS. Also the position of the contractor is to serve as the senior designee for Religious Education for the community through a week-to-week coordination and administration for the Catholic religious education needs provided to both the military members and their dependents at Lajes Base Chapel. This Position requires implementing and designing programs for adults, youth, and children, as well as to assist in building a solid foundation for spiritual growth in Catholicism. Contractor will work closely with the Chaplain in charge of the Catholic Parish, Chaplain Assistant, Catholic Parish Council, lay volunteers, parishioners and other chapel staff. Please reference the attached statement of work (SOW) for further information. QUALIFICAIONS: Must have a strong understanding and knowledge of Catholic faith and be able partake in its full sacramental life. Also a basic understanding of Canon Law with respect to the day-to-day aspects of the practice of liturgical, catechetical, pastoral work of a Catholic Community and the expectations of every Catholic as seen fit by the Catholic Chaplain. Shall have the ability to work with the pluralistic environment of the Air Force Chaplain Corps, as well as, with others, including: Chaplains, Chaplain Assistants, other contractors, volunteer staff, and the Catholic and Protestant Community members. Must be able to speak and correspond fluently in English.Shall be subject to an extensive background check/Child Care National Agency Check and Inquiries (CNACI). A negative CNACI shall result in the immediate termination of the contract.Individual will be required to receive Information Assurance (IA) training prior to use of government computers.Computer skills to include Microsoft Word, Microsoft Outlook, PowerPoint and Excel spreadsheets are required.Must be able to demonstrate qualifications in Catholic Religious Education. Contractor must personally perform the services, or if not available; contractor must subcontract duties. The subcontract must be performed be performed by someone of equal or higher proficiency. Also, must be approved by the contracting official prior to services being rendered. WORKING HOURS/DAYS: This position shall be accomplished on a weekly basis through the dedication of 30 office hours Wednesday - Sunday each week to provide for sufficient customer service, in building T-305. This position will require work on Saturdays and Sundays. The actual work schedule will be set up in collaboration with the Chaplain in charge of the Catholic Community.Duties shall take place at the Base Chapel (Bldg. T-305). All other locations shall be by approval of the Chaplain in charge of the Catholic Community and/or Wing Chaplain. (see attached Statement of Work for further Details) FOB: Destination (vi) See paragraph (v). And attached Statement of Work. (vii) Place of Delivery: Lajes Field, Azores, Portugal. The following provisions and/or clauses apply to this acquisition. (viii) Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items. Offerors must provide pricing on attached Bid Schedule (Attachment). Offerors must provide qualifications based on the Statement of Work (Attachment) for the service required. ADDENDUM to 52.212-1 PREPARATION INSTRUCTIONS A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions herein. The proposal must be complete, self-sufficient and respond directly to the requirements of this request. The response shall consist of two (2) parts; Part I - Price Proposal and (2) Part II - Technical B. Specific Instructions: 1. PART 1 - PRICE PROPOSAL - Submit original copy (a) Bid Schedule: Completed Bid Schedule. (b) Price shall be submitted in Dollars. 2. Part II- TECHNICAL - Submit 1 copy (a) Ability to meet requirements of Statement of Work. (ix) FAR 52.212-2 Evaluation-- Commercial Items ( Jan 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: 1: Price. ADDENDUM TO 52.212-2 BASIS FOR AWARD Quotes will be evaluated in accordance with FAR 13.106-1, which will be based on price and technical. A comparative evaluation between offers will be conducted. The Government will award the contract to the offeror who represents the "Best Value" in terms of technical capability and price. A. Price Evaluation: The offeror's proposed prices will be determined by total price provided for on the price list. The price evaluation will document the reasonableness and affordability of the proposed total evaluated price based on the following subfactor: -Completed price list. B. Technical Capability: The offeror's ability to meet the requirements as described in the Statement of Work. The proposals shall be evaluated against the following subfactor: -Qualifications C. Offerors are cautioned to submit sufficient information and in format specified in ADDENDUM 52.212-1 INSTRUCTIONS TO OFFERORS. Offerors may be asked to clarify certain aspects of their proposal. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.
