Description
Synopsis: The Michigan Army National Guard is seeking physicals and testing services for our Lansing and surrounding area technicians in accordance with Governmental Reporting Standards. The Contractor shall perform all work at the Contractors Facility. The Anticipated performance period is expected to span no less than 3 years and no longer than 5 years. Service calls will be performed on a "on need" basis, with calls placed with indefinite quantities and delivery dates. Services will be billed individually with pre-established mutually agreed upon cost per service, and not to exceed an annual aggregated amount which has yet to be determined. Scope of work to include but not limited to; Technician Physicals; to include height, weight, temperature, blood pressure (sys. & dias), pulse, dental defects and disease, distant vision, near vision, color vision, amsler grid for laser workers. Abnormality checks to include; head, face, neck and scalp, nose, sinuses, mouth and throat, ears - general, eyes - general, drum (perforation), Ophthalmoscope, pupils (equality and reaction), ocular motility, heart, lungs and chest, vascular system, anus, rectum, and prostate, abdomen and viscera, external genitalia, upper extremities, lower extremities, feet, spine, other musculoskeletal, identifying body marks, scars, tattoos, skin, lymphatics, neurologic, psychiatric, pelvic. Lab work to include: PSA, Urinalysis, Over age 40 Fecal Occult, Heavy Metals (to include Lead, Mercury, Arsenic, Cadmium), Comprehensive Metabolic Panel, CBC W/Diff, ZPP, Beta 2 Microglobulin Urine Respiratory Clearance to include; OSHA questionnaire, PFT Chest X Ray to include, PA /LAT 2 view (asbestos) Audiometric examinations shall include at a minimum: Informal otoscopic examination bilaterally. Pure tone air conduction hearing threshold determination to include Test frequencies: 500, 1000, 2000, 3000, 4000, 6000 Hz. Bilateral thresholds shall be obtained at each frequency. Audiometers, calibrated to the International Standards Organization (ISO 1964) or the American National Standards Institute (ANSI 1969), will be used to obtain pure tone thresholds. All audiometric tracings or audiometric readings recorded on reports of Medical examination or other medical records will be clearly identified. Audiometer Test Performance Must be performed by a licensed or certified audiologist, otolaryngologist, or other physician; or by a technician certified by the Council for Accreditation in Occupational Hearing Conservation or who has completed equivalent military training. A technician who performs audiometric test shall be responsible to an audiologist, an otolaryngologist, or other physician. Standard instructions shall be given to individuals before testing. Test must be conducted in a testing environment with background octave band SPLs not greater than the following: For 500 Hz, 27 dB., For 1000 Hz, 29 dB., For 2000 Hz, 34 dB., For 4000 Hz, 39 dB., For 8000 Hz, 41 dB. The test environment shall be resurveyed annually using equipment conforming at least to the Type 1 requirements of the latest approved ANSI Standards S1.4A to S1.4-1983 and the order 3 extended range requirements of the latest approved ANSI Standard S1.11-1986. Contractor shall submit invoice monthly or upon expiration of agreement. Invoice should include contractor name, agreement number, date of call, call number and or name of personal for which the service is provided, itemized list of services to include cost of each service less any discounts and date of service. Call will be considered completed and billable upon total completion of all requested services per individual reported to requestor in proper format, to include; use of mandatory forms, required test and procedure forms, DD form 2807-7, DD Form 2808, OSHA questionnaire, Supervisor's respirator use risk assessment, DD form 2215, and DD Form 2216.
