- The Erie VA Medical Center has an upcoming requirement for Artwork Design Services. The Statement of Work can be viewed below. All responses should be sent to Jennifer Balsiger at jennifer.balsiger2@va.gov no later than November 4, 2011. SCOPE OF WORK FOR ARTWORK SERVICES AT THE ERIE VAMC The Erie VA Medical Center (VAMC) has a requirement for artwork services at the facility. The Government intends to award firm fixed price contract in response to this requirement. The successful offeror confirms that they are located within a 150 mile radius of the medical center OR they confirm that they are willing and able to make multiple trips to the Erie VAMC for meetings, site inspections, etc. Scope General The Contractor shall provide custom design, product, and installation of artwork and interior accessories throughout the Erie VAMC and associated Community Clinics through the contracted service period. The selected firm shall have proven experience in providing design and installation services for artwork and interior accessories. Services Provide: Professional Design Services, Products & Installation to include but not limited to: "Creating design and installation plans for the Erie VA Medical Center (VAMC), including: oSpecifications of products to be installed oDetailed location plan and installation instructions for products oGraphical representations/digital images of products oTraining/instructions to the Erie VAMC in the maintenance of all installed product "Site investigation - architectural, structural, and electrical as applicable "Products included in this scope of work are: oFramed, canvas, and custom art oCustom framing of VA supplied art and documentation oMurals and vinyl wall applications oSculptures, seals, flags, and plaques oAccessories and silk plants This contract involves professional art design, art program development, and installation services. The Contractor shall perform the following: "Develop and install artwork and accessories as needed throughout the Erie VAMC facilities "Submit a minimum of three (3) design options to the Erie VAMC for each project (delivery/task order) oPresent the designs options to the VAMC Interior Designer "Prepare and present samples including: oPhysical piece/portion of frames, materials, and product to determine tangible product quality oElevation views of each design option as installed in defined project area oComplete specifications for the product being specified including floor plans, mounting locations/heights, and mounting instructions "Install all artwork and accessories per the specifications oInstallation times will vary based on location and accessibility oICRA precautions will need to be met when creating dust and debris oAt completion of installation, the firm will make a final inspection/walk through with the Erie VAMC Interior Designer and prepare a report noting deficiencies and corrections necessary for completion of the project "Provide Erie VAMC with location plans and product information, including pictures, dimensions, and care instructions, for each installation. A format will be agreed upon at the start of the contract Site Investigations The Contractor shall: "Make site visits as necessary to determine existing, as-built conditions affecting the project "Field-verify all dimensions and aspects that relate to the project "Coordinate site visits with the Erie VAMC Interior Designer "Document existing conditions Note: Information provided to the Erie VAMC will be used as reference only. Design Review Schedule The Contractor shall submit review packages for each project so they are received on the noted day or earlier. Each project will demand a different timeline for installation and the firm needs to be flexible with lead times as each project dictates. Example Submissions Schedule: Initial Project Meeting (Phase 1)0 days Preliminary Design (Phase 2)5 days PDT Review Meeting (Phase 2)10 days Final Design (Phase 3)15 days PDT Review Meeting (Phase 3)20 days Installation (Phase 4)45 days Contract Documents50 days Design Requirements/Submissions The Contractor shall deliver options and project specifications packages directly to the Erie VAMC Interior Designer on the established dates in order to allow for review by the VA. The following information shall be included in each submission as attachments. Phase 1: Planning Project begins with an initial Planning Meeting with the Contractor, Erie VAMC Interior Designer, and the Contracting Officer. Tasks to be completed include: "Review the responsibilities of the Contractor and Erie VAMC regarding input, design, documentation, coordination, direction, and approvals "Establish a project schedule which relates to the project's sequence of events and priorities of importance "Discuss functional relationships regarding design, implementation, document submissions, and installation plan "Survey and document existing conditions and overall dimensions "Deliverables include: oPreliminary project schedule oOne (1) electronic file in PDF format Phase 2: Preliminary Design The Contractor will develop and present three design options for the current project. "Present the design schemes to the Erie VA Interior Designer and designated staff for their review and comment "Deliverables include: oElevation views of each design option as installed in defined project area oThree-dimensional graphical representation/digital images of each design option, as appropriate oSamples of materials and products to be used Phase 3: Final Design The Contractor will submit/verify final design and product selection, as well as revise plans and specifications. "Documents will be presented to the Erie VAMC Interior Designer and designated staff "Deliverables include: oElevation views of each design option as installed in defined project area oThree-dimensional graphical representation/digital images of each design option, as appropriate oInstallation plan, instructions, and mounting locations oInstallation schedule; one hard copy and one electronic file in PDF format Phase 4: Installation The Contractor shall install all products per the contract plans and specifications. "Installation will be accomplished with little or no disruption to the Erie VAMC daily operations "Post installation, the Contractor will oMake a final inspection and walk through with the Erie VAMC Interior Designer and prepare a report noting deficiencies and corrections, as required, for the successful completion of the project oParticipate in final acceptance inspection, as scheduled, and generate an "items for completion" list "The Contractor shall provide Erie VAMC with location plans and product information, including full color pictures, dimensions, and care instructions, for each installation. To complete the training modules: "Access courses electronically through EES at the following website: https://www.ees-learning.net/librix/loginhtml.asp?v=librix. oAfter you have gained access to the site, follow procedures to register if you are new user, login using your username and password oClick "Available Courses" (left hand column) oEnter FY11 in the "Keyword" from the Advanced Search section (center of page) oClick "Search" oSelect VA Privacy and Information Security Awareness and Rules of Behavior FY 11. oPrint the training certificate at the end of the session or; "Review the text version of the training modules and complete the certificates found at the end of the training modules. "The contractor shall provide to the VA COTR a copy of the completed training certificate(s).
