Description
FY11 FACILITY IMPROVEMENTS 554-11-812 Denver VA Medical Center Scope of Work-REV 8/19/11 Contractor to provide all labor, supervision, tools, material, equipment and design-build services necessary for design and construction period services of a multi-scope project as listed below. 1)ELEVATOR MACHINE ROOM CORRECTIONS: -Install gate in elevator machine room, 11C100, in order to meet 29 CFR 1910.212(a)(1). Gate must be full height and run entire length of room. (Reference Drawing A-100) 2) HAZARD LEVEL 2 ABATEMENT: -Complete full asbestos abatement of the following areas listed: a. Approximately 500 linear feet of window putty in M&R shops, building 6 (Reference Drawing A-101) 3)LAB HOOD: Prepare space in room 3A141 for newly purchased Nuaire fume hood, Model NU S435 400. VA has already purchased equipment. Outside vendor will provide installation services of new equipment. Contractor is responsible to prepare the space for the new unit to be installed. Contractor to complete work in accordance with all local code regulations and will consist of electrical, HVAC, plumbing and architectural renovations. New fume hood will be installed in space of an existing fume hood in the lab. Contractor will be responsible for design and removal of existing lab hood and final install (connections) of new hood. Work will require installation of new ductwork and exhaust fan to the exterior of the building. Required exhaust of new fume hood is 754 CFM and current exhaust is only 686 CFM with fan at full speed. New fume hood also comes with a motorized base and 12" of flex duct to allow for adjustable height settings Equipment specs/install guide attached (Attachment A) 4)BB122a HVAC: -Disconnect existing HVAC supply in BB122a from BB122b. Install new 1-hr. smoke wall where supply is removed as required. Fire caulk as required for fully rated wall. -Provide new HVAC supply to room BB122a from BB122. Route new ductwork to existing diffuser. Test and balance fan coil unit. (Reference Drawing A-102) 5)PROSTHETICS WAITING ROOM WINDOW: -Install ADA accessible reception 30"x36" window, frame and shelf in prosthetics waiting room. Patch and paint walls as required to match existing. Window to be sliding tempered glass with lock (Reference Drawing A-102) 6)DENTAL & 4SOUTH PAINTING: -Patch and paint patient rooms on 4South to include one accent wall in each room. Colors to be chosen by VA Medical Center Interior Designer. -Patch and paint exam rooms and alcoves in Dental. This includes both alcoves with patient exam rooms attached. Refer to drawing for additional areas in scope. Paint colors to be chosen by VA Interior Design Department. -DEDUCTIVE ALTERNATE: Contractor to patch/paint large lab exam room, 3B144, in Dental. Work in this space will need to be completed on a weekend. (Reference Drawing A-103 AND A-104) 7)7E SINKS REMOVAL: -Remove 2 existing sinks in hallway on 7North and 7South psychiatric unit. Cut and cap lines and patch and paint walls to match existing. (Reference Drawing A-105) 8) OI&T DUMBWAITER VENTILATION: -Investigate feasibility of venting main OI&T riser closet. Provide fire code review of 2 hour enclosure and determine options to vent riser to prevent excessive build-up and loss of equipment. 9) MAIN STEAM SUPPLY SUPPORTS: -Add frame near corner of bldg. 24 steam supply supports and beams spanning from new frame to existing frame near the corner of building 1. (Reference Drawing S-100) 10)OR RENOVATIONS: -Room 4A110B a. Remove existing Steris unit and cut and cap plumbing as required. Remove all existing cabinetry and sinks from north wall and all shelving on west wall. Cut and cap plumbing as required and patch and paint all walls to match existing. Remove square box fixed to floor and repair flooring as needed. Install upper cabinets with doors on the south wall. Install strip of electrical outlets on north wall. -Room 4A121A a. Remove existing sink and install new stainless steel sink with counters on either side. Remove existing shelving on south wall and install new "L" shaped upper cabinets with doors on the east and south walls for storage. Place one small shelf below cabinets on south wall. Install telephone outlet and duplex outlet. Patch and paint all walls to match existing. -Room 4A102 a. Remove (2) existing Steris units and cut and cap plumbing. Remove existing sink and cut and cap all exposed piping behind wall. Disconnect existing steam sterilizer. Patch and paint all walls to match existing. (Reference Drawing A-106) DESIGN CRITERIA -Contractor shall determine phasing to ensure the least disruptive construction schedule and design the subsequent work accordingly. -Specifications will be developed using the standard VA specifications. These and other VA design criteria can be obtained at http://www.cfm.va.gov/TIL -The drawings will be produced using industry standard details and VHA CADD Deliverable Guidelines, on a VA provided title block. All drawings will be standard, scaled, e-size (30" x 42") drawings or half size drawings. Contractor will be provided with architectural background drawings in AutoCAD 2010. DB A/E shall confirm in the field, actual as-built conditions. - Contractor to provide phasing and coordination of any utility shutdowns. - Contractor is responsible for survey of existing conditions. Any drawings provided shall be used only as a guide and all information must be confirmed by DB A/E. - Contractor to stamp drawings as to which review (65%, 95%, or Construction). Also, note on review drawings "NOT FOR CONSTRUCTION." SCHEDULE Design time allotted to be as follows: -VA receives 65% working drawings and specifications- 6 weeks. - Comments by VA back to A/E- 8 weeks. - VA receives 95% working drawings and specifications- 12 weeks. - Comments by VA back to A/E- 13 weeks. - VA receives 100% construction drawings and specifications- 14 weeks. Construction items to be completed within 150 days of VA approved final construction drawings. Design/Build AE to work with COTR on construction scheduling and phasing of the project. Work can be completed in multiple areas at one time. Dental and 4S painting as well as OR and lab hood installation will require phasing and possible off-hours work. Design/Build A/E to work closely with VA COTR and Infection Control to ensure all infection control requirements are fully incorporated into the project design documents. HOURS OF WORK Schedule work in advance with COTR. All work shall be complete during normal business hours, Monday through Friday 7:00 am - 4:00 pm, excluding Federal Holidays. Some areas may require off-hours work and will need to be scheduled in advance with COTR.
