- Shipping addresses This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR part 12 and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and another written solicitation will not be issued. (ii) This is a request for quote under solicitation number HS0021-11-Q-0243 (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-49 dated 1/31/2011. (iv) This is a 100% small business set aside under NAIC 423430.. (v) Requirement is for Secure laser fax machine Ilex TEMPEST, model number #6200-SF; w/STE/STU-III cable, 9000 page supply cartridge, cimpliance to MIL-STD-188-161D, 128Kbps data for STE, Asynchronous Data for FNBDT terminals. Contractor's should include minimum one year maintenance 24/7. It will be competed under simplified acquisition. Quantity 26 for various locations. See attached shipping locations. (vii) Date(s) and place(s) of delivery and acceptance and FOB point are available in the solicitation document. (viii) A statement that the provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. The clause has been tailored to meet the specific requirements of the agency. The clause is contained in full text in the solicitation document. (ix) A statement regarding the applicability of the provision at 52.212-2, Evaluation -- Commercial Items, is applicable: The evaluation factors are Conformance to Specifications, Experience, Past Performance, and Price. Lowest price Technically acceptable. (x) Include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer, unless they have been completed online at http://orca.gov. If they have been completed online, submit only section "b" of the provision. (xi) Clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
