- FY ' 12 1800 Protestant Contemporary Worship Choir Director PWS FY '12 1800 Protestant Contemporary Worship Musician PWS FY '12 1130 Protestant Gospel Service Musician PWS FY '12 1130 Protestant Gospel Choir Director PWS FY '12 0900 Protestant Praise Service Worship Leader PWS FY '12 - 0900 Protestant Praise Service Musician PWS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, is being issued as a AndersenAFB_ChapelServices Request for Quotation (RFQ) for the 36th Wing Chapel. All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 711510 and the small business size standard is $7.0 M. The following commercial items are requested herein: 1130 Gospel Service Musician CLIN 0001: Price per service unit_________. In accordance with the attached Performance Work Statement (PWS) 1130 Gospel Service Musician, Period of Performance (POP) 8 Oct 2010 - 30 September 2011. 1130 Gospel Service Choir Director CLIN 0002: Price per service unit________. In accordance with the attached Performance Work Statement (PWS) 1130 Gospel Service Choir Director, Period of Performance (POP) 8 Oct 2010 - 30 September 2011, billable as Service 0900 Protestant Praise Service Musician CLIN 0003: Price per service unit________. In accordance with the attached Performance Work Statement (PWS) 0900 Protestant Praise Service Musician, Period of Performance (POP) 8 Oct 2010 - 30 September 2011 0900 Protestant Praise Service Music Director CLIN 0004: Price per service unit________. In accordance with the attached Performance Work Statement (PWS) 0900 Protestant Praise Service Music Director, Period of Performance (POP) 8 Oct 2010 - 30 September 2011 1800 Contemporary Worship Musician CLIN 0005: Price per service unit ________. In accordance with the attached Performance Work Statement (PWS) Catholic Parish Coordinator, Period of Performance (POP) 8 Oct 2010 - 30 September 2011. 1800 Contemporary Worship Service Music Director CLIN 0006: Price per service unit________. In accordance with the attached Performance Work Statement (PWS) Catholic Religious Education Coordinator, Period of Performance (POP) 8 Oct 2010 - 30 September 2011 How to Submit Quotes Offerors must submit quotes on all of the six line items. Each quoted price must be clearly identified to the CLINs above. To submit a quote: 1. Fill in the price per service unit for each and every position quoted in the space reserved for the price per unit for the respective position in the format provided above. And 2. Provide supporting documents sufficient to demonstrate qualifications (Work History and/or Past Performance Information and/or Resume, etc.). And 3. Submit the price quote information and supporting documents via email to SSgt Bardia Barmaki at bardia.barmaki@andersen.af.mil, if needed copy Mr. Keith Beem at keith.beem@andersen.af.mil. Responses to this RFQ must be received via e-mail not later than 6pm eastern standard time on Tuesday 4 October 2011. Oral Quotes will not be accepted. All firms must be registered in the Central Contractor Registration database @ www.ccr.gov to be considered for award. Services are to be delivered FOB destination at Andersen AFB Guam 96929. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items apply. Proposals will be evaluated and awarded to the offeror with the proposal representing the best value to the government. Award for each of the fourteen solicited positions will be based on experience and price. Experience is more important than price.
