- Attachment 2 - Wage Determination 05-2517 Attachment 1 - Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number for this procurement is FA4855-11-Q-0197 and is a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-50, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20110317, and Air Force Acquisition Circular (AFAC) 2011-0214. This is a 100% Small Business Set-Aside. The North American Industry Classification System (NAICS) is 621512 Diagnostic Imaging Centers. The Small Business Size Standard is $12.5M. The offeror must quote on an all or none basis. Award will be based on Best Value to the Government. Written quotes are required (oral offers will not be accepted). Only one contract will be awarded. BID SCHEDULE: 0001 OFF-SITE TELERADIOLOGY SERVICES. (Non-Personal Services) Contractor to furnish all labor, tools, parts, materials and facilities necessary to provide offsite teleradiology interpretation services by qualified radiologists in accordance with the PWS (Attachment 1) for Cannon AFB, NM. PERIOD OF PERFORMANCE: 1 Oct 11 to 30 Sep 12. The following factors shall be used to evaluate offers: (i) technical capability to meet the Government requirement; (ii) past performance (see FAR 15.304); Contractor is to submit three past performance references (POC name, agency name, contract number, description of services performed, dollar value, period of performance) for contracts performed within the last three years for services similar in scope to the requirement. (iii) price; Technical and past performance, when combined, are more important than price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of Provision) Addendum to FAR 52.212-2 Evaluation-- Commercial Item The provision is tailored to add the following paragraphs. (d) Basis for Award. First, each proposal will be evaluated for technical acceptability on a pass/fail basis. After a proposal is determined technically acceptable, price and past performance will be evaluated. The government will award to the technically acceptable offer considered to be the offeror who will provide the best value to the government. The government reserves the right to award a contract to other than the lowest evaluated price if that offer is not considered to present the best value to the government. (e) Technical Evaluations. The technical evaluation process will evaluate the offeror's capability/qualifications for the requirement as detailed in the PWS (Attachment 1). The offeror's technical information submission will be evaluated as an overall acceptable (pass) or unacceptable (fail). Technical information will be evaluated on the three (3) subfactors listed below. If any subfactor results in an unacceptable rating, the entire technical final rating will be unacceptable. Only offers determined to be technically acceptable, initially or as a result of clarifications, will proceed to be considered for award. Subfactor 1 - Contractor Qualifications. See standards listed in the PWS (Sec. 4.7.2 and 4.7.3) Subfactor 2 - List of Compatible Equipment. The Government will evaluate the list of equipment provided by the Offeror to determine if equipment will be compatible with Government equipment in order to provide teleradiology services in accordance with the standards of the PWS (Sec. 3.2.1.). Subfactor 3 - Compensation Plan for Professional Employees. The Government will evaluate each plan to assure that it reflects a sound management approach and understanding of the contract requirements. (f) Price Proposal Evaluations. Price will be evaluated to determine the offeror's understanding of the contract requirements as expressed by the solicitation. Any inconsistencies (whether real or apparent) in price must be clearly justified. The burden of proof for credible pricing rests solely with the offeror. Pricing will be considered complete when the offeror has proposed firm-fixed prices for all items listed on the Price List (See end of combined synopsis/solicitation). Incomplete pricing will be deemed non-responsive and will not be accepted. (g) Past Performance Evaluations. The contracting officer shall seek relevant performance information on offerors based on the past and present efforts provided by the offeror and data independently obtained from other government and commercial sources. The government reserves the right to seek information on higher priced offerors if none of the lower priced offerors receive a Substantial Confidence performance confidence assessment rating. The purpose of the past performance evaluation is to assess the offeror's ability to perform the effort described in the solicitation, based on the offeror's demonstrated present and/or past performance. The assessment process will result in an overall performance confidence assessment rating as defined in AFFARS Mandatory Procedures 5315.3, Table 3. Offerors with no relevant past or present performance history shall receive an Unknown Confidence rating, meaning the rating is treated neither favorably nor unfavorably. Confidence Assessment Ratings Substantial Confidence - Based on the offeror's performance record, the government has a high expectation that the offeror will successfully perform the required effort. Satisfactory Confidence - Based on the offeror's performance record, the government has an expectation that the offeror will successfully perform the required effort. Limited Confidence - Based on the offeror's performance record, the government has a low expectation that the offeror will successfully perform the required effort. No Confidence - Based on the offeror's performance record, the government has no expectation that the offeror will be able to successfully perform the required effort. Unkown Confidence - No performance record is identifiable or the offeror's performance record is so sparse that no confidence assessment rating can be reasonably assigned.
