Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. It is the Government's intent to solicit, negotiate and award a contract for a "Brand Name" Requirement (FAR 52.211-6). The requirement is for Streck Labs (manufacturer), ESR Controls, Level 1 & 2, 12x9ML DS, product number 214112 and Streck Labs (manufacturer), ESR VAC Tubes, 1.2ML 100/BX, product number 240360 to be ordered as needed for the Dept. of Pathology, Womack Army Medical Center (WAMC), Fort Bragg, NC 28310. No equals will do, must be "Brand Name", product is written into a Standard Operating Procedures (SOP) for the Laboratory. ESR CHEX (LEVEL 1&2) 12X9ML DS: 12x9.0ml (Level 1 & 2) ESR-Chex is a two-level hematology control used to monitor erythrocyte sedimentation rates. ESR-Chex is manufactured from human red blood cells, and verifies the precision and accuracy of manual and automated methods. ESR-Chex is assayed for the most frequently used sedimentation rate tubes in conjunction with the Classical Westergren, Modified Westergren and Wintrobe methods. Automated and modified procedures do not require removal of sodium citrate, and ESR-Chex is used. ESR VAC TUBES 1.2ML 100/BX: 1.2ml (Box of 100) ESR-Vacuum Tubes are evacuated tubes used for the collection of venous blood. ESR-Vacuum Tubes are used to transport and process blood for testing the Erythrocyte Sedimentation Rate (ESR) of whole blood in the clinical laboratory. ESR-Vacuum Tubes are available in 1.2ml and 2.0ml vacuum draw configurations, and are designed specifically for use with the ESR-100 and ESR-Auto Plus automated sedimentation rate analyzers. Interested persons may identify their interest and capability to respond to the requirement by submitting proposals/quotes. All proposals/quotes received within the date of publication and closing of this solicitation will be considered by the Government. All responses must be received by September 19, 2011, 12:00 AM Eastern to be considered responsive. Responses must be submitted via email only to mary.a.martin@us.army.mil. Quotes/Proposals need to filled in with a per line price using the attached SF1449. Be sure to provide your company's Dunns and/or Cage Code along with quote. Be sure to fill in price for each option year, ie. Item 1001 through 4002.
