Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number VA-246-11-RQ-0873 is issued as a Request for Quotation (RFQ) on an unrestricted full and open competitive basis. The resulting order will be awarded as a Firm-Fixed Price with Economic Price Adjustment Purchase Order. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. The associated North American Industry Classification System (NAICS) Code is 621111 with a small business size standard of $10 Million. The Department of Veterans Affairs, VISN 6, Centralized Acquisition Service has a requirement for a 3 month period of performance for Off-Site Radiation Oncology Therapy Services for eligible Veterans Affairs (VA) Beneficiaries at the Asheville VA Medical Center, Asheville, NC, in accordance with the terms and conditions stated herein. All examinations/procedures including appropriate follow-up services are performed on an outpatient basis at the Contractor's facility. The Contractor's facility shall be located within a seventy (70) mile radius of the Department of Veterans Affairs Medical Center, 1100 Tunnel Road, Asheville, NC 28805, for the convenience of patients in the surrounding area. The period of performance for all work awarded under this order is 10/1/2011 through 12/31/2011. Interested offerors capable of furnishing the government with the requested service listed in the attached RFQ for this synopsis should submit a quotation to the email address listed below. It is the responsibility of the offeror to read the RFQ in its entirety and follow all the instructions for submission of a quotation. Incomplete quotes will be considered non-responsive and not considered for award. Quotations must be submitted via electronic mail to: Sandra.bennett3@va.gov or via Fax Number 757-728-3132. Quotations are due by 1:00PM EST, Monday, September 26, 2011 to the attention of Sandra Bennett, Contracting Officer.
