Description

Questions about the solicitation ¦. 1.Can annual surveys of a contractor's particular program be submitted for the past performance portion? 2.May a contractor list a tentative facility as the location and then change at a later date if needed? 3.If applying to all three Mental IOP Beds solicitations, is it required for three different sets of past performance surveys to be submitted? Or will three surveys suffice for all three solicitations? 4.Who fills out the surveys? Actual past veterans or agencies to which services were provided? Answers to the questions ¦ 1.No, there is a survey attached at the bottom of the word document of the solicitation labeled past performance survey. 2.The location listed, will be the location for inspection before contract award. 3.Reference all solicitation numbers, which the Contractor is providing a quote for, on the past performance surveys. Therefore the Contractor will have provided one set for all solicitations applicable. 4.All surveys will have to be submitted to the Contracting Officer by 5p on 9/20/2011 from the individual(s) of the agencies to which services were provided.
