- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. The associated North American Industrial Classification System (NAICS) code for this procurement is 541519 (Other Computer Related Services); Size standard is $25M. This RFQ is being issued as Firm Fixed Price, 100% Set-Aside Small Business. All responsible sources may submit a quotation which shall be considered by the agency. A Laboratory Management System for General Leonard Wood Army Community Hospital, MO, FOB Destination. All Contractors shall provide an offer for the following CLINs, CLIN 0001, Laboratory Management System: The contractor shall provide the Laboratory Management System (LMS), equipment, maintenance, technical support and service. The LMS will be located at General Leonard Wood Army Community Hospital, MO. See salient characteristics for details. Invoice; Quantity: 1 Each. Period of Performance: 24 Sept 2011 thru 23 Sept 2012. OPTION CLIN 1001: Laboratory Management System: The contractor shall provide the Laboratory Management System (LMS), equipment, maintenance, technical support and service. The LMS will be located at General Leonard Wood Army Community Hospital, MO. Invoice; Quantity: 1 Each. Period of Performance: 24 Sept 2012 thru 23 Sept 2013. OPTION CLIN 2001: Laboratory Management System: The contractor shall provide the Laboratory Management System (LMS), equipment, maintenance, technical support and service. The LMS will be located at General Leonard Wood Army Community Hospital, MO. Invoice; Quantity: 1 Each. Period of Performance: 24 Sept 2013 thru 23 Sept 2014. OPTION CLIN 3001, Laboratory Management System: The contractor shall provide the Laboratory Management System (LMS), equipment, maintenance, technical support and service. The LMS will be located at General Leonard Wood Army Community Hospital, MO. Invoice; Quantity: 1 Each. Period of Performance: 24 Sept 2014 thru 23 Sept 2015. OPTION CLIN 4001: Laboratory Management System: The contractor shall provide the Laboratory Management System (LMS), equipment, maintenance, technical support and service. The LMS will be located at General Leonard Wood Army Community Hospital, MO. Invoice; Quantity: 1 Each. Period of Performance: 24 Sept 2011 thru 23 Sept 2012. The Government will award a contract resulting from this combined synopsis/solicitation using lowest price, technically acceptable. Technically acceptable quote must meet all salient characteristics and must be able to comply with the Business to Business Partner Provision.
