Solicitation Number: VA-260-11-RQ-1193 Notice Type: Combined Synopsis/Solicitation Added: August 18, 2011 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The reference/solicitation number is VA-260-11-RQ-1193 and the solicitation is issued as a request for quotation (RFQ). The combined synopsis/solicitation will result in a contract and corresponding purchase order. The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The product numbers listed above are specific to Stryker Corporation. The salient characteristics of product being purchased are: PRODUCE PURE HD 1080i IMAGING “ MAINTAIN AND SUSTAIN HD FORMAT THROUGHOUT THE ENTIRE VIDEO CHAIN/SYSTEM WHICH INCLUDES ALL TOWER COMPONENTS: OPTOCS, CAMERA HEAD, CCU, IMAGE MANAGEMENT SYSTEM EQUIPMENT, AND MONITOR “ MEETS THE ATSC BROADCAST STANDARDS FOR HDTV “ HD WIDESCREEN CCD (CHARGE COUPLED DEVICE) DESIGNS FOR NATIVE ACQUISITION OF THE DIGITAL HDTV IMAGE “ MUST HAVE A 3-BUTTON, 6-FUNCTION INTERFACE CONTROL IMAGE AND REMOTE OPERATION OF COMPATIBLE PERIPHERAL DEVICES LIKE CCU ™S, PRINTERS, AND IMAGE MANAGEMENT SYSTEMS = LIGHTWEIGHT DISPLAY “ MUST HAVE A LIGHT SOURCE WHICH CAN SELF REGULATE WHEN ATTACHED TO AN OPERABLE SCOPE “ MUST HAVE A COMMERICAL GRAGE COLOR PRINTER “ WARRANTY SHALL BE ONE YEAR, PARTS AND LABOR MIMIMUM. TRAINING/IN SERVICE (APPLICATIONS); COST OF TRAINING IF ANY IN CONJUNCTION WITH PURCHASE “ AVAILABILITY OF VENODR TO PROVIDE SERVICE WITHIN 24 HOURS. OTHER: PROVIDE DOCUMENTATION OF GOVERNMENT FACILITIES WHERE THIS EQUIPMENT IS ALREADY IN USE, IDENTIFY REFERENCES WITH POINTS OF CONTACTS AND TELEPHONE NUMBERS. Delivery and acceptance of deliverables will be FOB destination within 30 days of receipt of order to VA Medical Center, 1601 E. 4th Plain, General Warehouse, Vancouver WA 98661. (vi) The provision at FAR 52.211-6 Brand Name or Equal applies to this requirement (vii) Delivery and acceptance of deliverables will be FOB destination within 60 days of receipt of order to Portland VA Medical Center, 3710 SW U.S. Veterans Hospital Road, Portland OR 97239. (viii) The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation and incorporate the following addenda to 52.212-1, 52.204-6 Data Universal Numbering System (DUNS) Number; 52.204-10, Reporting Executive Compensation & First-Tier Subcontract Awards (JUL 2010) (Pub. (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items applies to this requirement. The Government will award a firm fixed price contract resulting from this solicitation. The evaluation factors for award are technical capabilities and cost with technical capabilities being evaluated as significantly more important than cost considerations.
