This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number to be used is VA-250-11-RQ-0351 and is being issued as a Request for a Quote (RFQ). The solicitation document and the incorporated provisions and clauses herein are those in effect through Federal Acquisition Circular 2005-26. The item being sought by this solicitation is: 1.Rock Salt for snow and ice control used on roadways, parking lots, and sidewalks. Quantity: 500 Tons. Potential vendors will be required to provide their own salt piler to distribute the requested material at the delivery site. Please provide a quote for the requested item. Include in your quote the following information: Delivery Charge, all applicable fees and line items, DUNS #, Tax ID number and fax number. Delivery will take place on or before the 30th day after the confirmation of the award, and the address will be the salt bin at the VA Medical Center at 4100 West Third Street, Dayton, OH, 45428. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). The North American Industry Classification System (NAICS) code for this procurement is 325188. This requirement is a Small Business set-aside and only qualified offerors will be eligible for award. The provisions at FAR 52.212-1, Instructions to Offerors - Commercial, FAR 52.212-4, Contract Terms and Conditions - Commercial Items, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, apply to this acquisition. Evaluation shall be based on price and price alone. Responses will be due to Tim Hunt, Contracting Specialist, no later than 12:00 P.M. EST Friday, September 16, 2011 and can be sent either via fax (937-262-5974) or via email (Timothy.Hunt@va.gov).
