Description

Further Clarifications to VA245-11-RQ-0179 Contractor shall be a Certified Industrial Hygienist (CIH) with over 10 years experience managing Safety and Industrial Hygiene programs exclusively in a healthcare setting, preferably the Veterans Affairs or another federal healthcare establishment. " Travel time within the 40 hours per week is not a consideration. Contractor is expected to be on-site starting 8am Monday and depart 430pm on Friday in consideration of a 30 minute non-paid lunch break. The past performance questionnaires must be completed and sent in with the offeror's proposal. 3 references are also requested and can be 3 additional references or the same 3 as used to complete the questionnaires. The questionnaires are due back along with the proposal. The date of August 4, 2011 is an error. Questionnaires are not due until the close of the RFP on 9/14/2011. Please submit your proposal by completing the first page (1449) along with your signature and by completing the Price Schedule B.3 for CLIN 0001 with a unit price hourly rate and an estimated total. Contractor is expected to be on-site starting 8am Monday and depart 430pm on Friday in consideration of a 30 minute non-paid lunch break.
