- Provisions and Clauses Minimum Specifications Buy American Act--Free Trade Agreements--Balance of Payments Program Certificate The Uniformed Services University of the Health Sciences (USU), located in Bethesda, MD, has a requirement for a pre-vac steam sterilizer (autoclave). The sterilizer will be used in the Laboratory Animal Management (LAM) facility to sterilize animal cages and accessories (water bottles, food bowls, etc), and related equipment. Additional information regarding USU and its mission can be found on our website: www.usuhs.mil. This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 - Acquisition of Commercial Items and FAR Part 13 - Simplified Acquisition Procedures. It is the responsibility of the contractor to monitor FedBizOpps (http://www.fbo.gov) for the issuance of any amendments to the solicitation. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-53. This requirement is set-aside for small businesses under NAICS code is 339113 (Surgical Appliance and Supplies Manufacturing), with a size standard of 500 employees. See attached provisions and clauses. Offerors must complete the DFARS 252.225-7035 Buy American Act--Free Trade Agreements--Balance of Payments Program Certificate and include a copy of the completed certificate with your quote. In order to be considered for award, offerors must be registered in the Central Contractor Registration (CCR) database (www.ccr.gov) and the Online Representations and Certifications Application (ORCA) database (www.orca.bpn.gov) by the close date of the solicitation. Quotes received from offerors not registered in both databases will not be considered. Offerors interested in responding to this requirement must provide a quote for the following items: CLIN DESCRIPTION 0001 AUTOCLAVE 0002 LOADING CART 0003 CARRIAGE 0004 REMOVAL OF EXISTING AUTOCLAVE 0005 INSTALLATION 0006 TRAINING 0007 FREIGHT Quotes for refurbished items will not be accepted. See attached minimum specifications for requirement details. The government will award a Firm-Fixed-Price (FFP) contract resulting from this solicitation to the responsible offeror whose quote, conforming to this announcement, will be most advantageous to the government. Award will be made on a best value determination considering price and non-price factors in accordance with FAR 52.212-2 - Evaluation -- Commercial Items Clause. Evaluation Factors (Listed in order of importance): 1. Price - Offerors must provide a price for each CLIN listed above. The resultant award will be a single FFP award; however for price analysis purposes the offeror must provide the following pricing information for CLINs 0004 (Removal/Disposal) and 0005 (Installation): • Number of labor hours/labor category • Number of individuals/labor category 2. Technical - Offerors must provide product literature, which demonstrates the capabilities of the quoted item(s). At a minimum the product literature must demonstrate that the quoted item meets the minimum specifications outlined in the specification sheet and must include a description of the operating procedures. Offerors must also provide a removal/installation/training plan. The plan, at a minimum, must include the following: • How many days after receipt of order (ARO) work can commence. • How many days removal of the existing autoclave will take? • How many days installation of the new autoclave will take? • What is included in on-site training (# of hrs, training material)? The government reserves the right to utilze technical and customer reviews as part of the technical evaluation. Offerors may provide quotes for multiple autoclaves. See below for applicable provisions and clauses for this requirement. The "nonmanufacturer rule" applies to this requirement. Per FAR 19.001, "the nonmanufacturer rule means that a contractor under a small business set-aside or 8(a) contract shall be a small business under the applicable size standard and shall provide either its own product or that of another domestic small business manufacturing or processing concern." A site visit is scheduled for September 14, 2011 (Wednesday) at 1:00 PM EST. Offerors interested in attending the site visit must email the contract specialist (Jody Bester - jody.bester@usuhs.mil) by COB Monday. USU is located on a secure installation and names must be added to the base access list. Offerors must provide the following information to attend the site visit: Name of Company Name of Attendee Email Address Telephone Number License Plate Number and State Site visit attendees should expect to arrive at the installation approximately 30 minutes prior to the scheduled site visit to allow for time to go through the gate and find parking. The site visit will start promptly at 1:00 PM EST. The site visit will begin in the Contracting Office, located in building A room 1040C. Offerors interested in attending the site visit are highly encouraged to use public transportation, as parking is limited. Every effort will be made to secure visitor parking, if needed. Directions to the installation and to USU from the main gate will be provided to attendees providing a valid email address. USUHS reserves the right to award without discussions. All questions must be submitted via email to Jody.Bester@usuhs.mil by close of business (COB) September 16, 2011. Responses to questions will be addressed as an amendment to the RFQ. No questions will be answered over the phone. Quotes shall be accepted by email only. No other method of submission will be accepted. Email all quotes to Jody.Bester@usuhs.mil. Quotes must be received by 5:00 PM EST on September 23, 2011.
