- Additional Documentation to be Completed by Vendor Representations and Certifications Sole Source Justification Standard Form 18 This notice is a combined synopsis/solicitation for commercial items (as defined by Federal Acquisition Regulation (FAR) 2.101) prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. The Government is seeking a quotation from Demand Technology Software, Inc. ; however, the agency will not issue a written request for quotation (RFQ). This acquisition is subject to FAR Subpart 13.1 regarding the application of simplified acquisition procedures. The solicitation number is SSA-RFQ-12-1029. The agency is issuing this solicitation as a non-competitive acquisition for Maintenance Renewal on the currently installed Demand Technology Performance Sentry software. This software is proprietary and Demand Technology Software, Inc. is the only source known to the government. There are no authorized re-sellers of maintenance support services. The period of performance is for 12 months. The place of performance is the SSA National Computer Center (NCC) at headquarters in Baltimore, MD. This maintenance support shall include all upgrades, updates, fixes and technical support. The government anticipates award of a firm fixed price purchase order. The Government will consider information received solely for determining whether to conduct a competitive procurement. Written responses to this notice shall contain sufficient documentation to establish a bona fide capability to fulfill this requirement. A determination not to compete this requirement is solely within the discretion of the Government. If the agency receives no responses, SSA will proceed with sole source negotiations. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. OBJECTIVE: The objective of this solicitation is to obtain Maintenance Renewal on the currently installed Demand Technology Performance Sentry software for the Social Security Administration's (SSAs) National Computer Center (NCC) in Baltimore, MD. SPECIFICATIONS SHALL INCLUDE THE FOLLOWING: Annual maintenance support for Sentry software. The Vendor is not required on-site. TERMS AND CONDITIONS: The provisions of FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (JUNE 2010) are incorporated by reference. FAR 52.212-5 applies as follows: (b)(4), (b)(21), (b)(24), (b)(25), (b)(26), (b)(27), (b)(29), (b)(36), (b)(40), (b)(45), (b)(48). INSTRUCTIONS FOR SUBMITTING QUOTATIONS: The provisions of FAR 52.212-1, Instructions to Offerors -- Commercial Items (JUNE 2008) applies and is hereby tailored as follows: Paragraph (a) The NAICS Code is 511210, business size standard $25 million. A completed copy of the representations and certifications at FAR 52.212-3 (MAY 2011) is required as part of the offer. The following additional information applies: Offeror must complete and include the attached ADDITIONAL INFORMATION DOCUMENT with quote. Vendors must submit their quote VIA EMAIL to the attention of Contracting Officer, Deborah R. Wilson at Debbie.R.Wilson@ssa.gov no later than 3:00 p.m. local prevailing time on Friday, September 9, 2011. You must submit your quote on a firm-fixed-price basis for both unit and total prices and shall reflect the same. EVALUATION CRITERIA: The provisions of FAR 52.212-2, Evaluation -- Commercial Items (JAN 1999) (Custom Tailored), for evaluation purposes, a purchase order award shall be made to the technically acceptable, lowest total price offeror. Vendors must submit all questions and submissions in response to this notice IN WRITING to the email address listed above. The Contracting Officer will NOT accept Telephone calls regarding this matter, nor will they be returned.
