This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number, VA-260-11-RQ-1583, is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective August 2, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 334516, Size Standard: 500 Employees. This procurement is set aside for Service Disabled veteran Owned Small Businesses (SDVOSB). ITEM DESCRIPTION OF QTY UNIT UNIT AMOUNT NO. SUPPLIES/SVCS PRICE 1 1.00 YR ___________________ ____________________ Shandon Excelsior Tissue Processor ONE-YEAR SERVICE AND MAINTENANCE AGREEMENT (GOLD PLAN WITH PM) for the period OCT 1, 2011 through SEPT 30, 2012 +++Service Contract Act, DOL WD 05-2439 (Rev.-12) applies++++ ***Contractor must be certified by the original equipment manufacturer for performing maintenance and service on this model of equipment.** Statement of Work Vendor is to provide full services and preventive maintenance functions for the Shandon Excelsior Tissue Processor in accordance with the manufacturer's recommendations as listed in the GOLD support agreement for the following equipment: 18201 Shandon Excelsior Tissue Processor, SN EX90900504 Full Service is to include One (1) Preventive Maintenance Visit. Unlimited On-Demand service call response and on-site repair services between the hours of 8:00AM - 4:30PM, Monday through Friday within 24 hours of notification. Provide all replacement parts and labor at no cost to the government. Programming, adjustments, instrument calibration, and repairs. Repairs to be performed either on-site or at Thermo Fisher Scientific. All shipping charges are covered for the instrument when applicable. Work is to be scheduled by the Laboratory Manager, Bio-Med, and Vendor. Contractor shall be responsible for: Providing a factory trained service technician. All travel expenses included. Supplying all equipment, materials, replacement/repair parts, and labor to accomplish preventive maintenance tasks per the manufacturer's recommendations. Supplying all required safety equipment. Keeping Lab equipment in first class condition. Clean equipment inside and out at each visit. Protecting veterans, visitors, and staff while working at VA Roseburg Healthcare System, Roseburg Campus. This involves preparing and presenting Safety Plan to the COTR & VA Safety Officer for review and approval prior to starting work on each job. Scheduling the work in advance with the POC during the window of Monday - Friday, 8:00AM - 4:30PM, excluding Holidays. There may be some situations that require working outside of normal business hours. Work outside of normal business hours requires COTR approval. Contractor will provide 24 hour notice prior to performing a maintenance visit. Vendor will notify Joseph Flammang, Bio-Med Tech at (541) 440-1000 EXT 45840 prior to and upon completion of work.
