- Please note that this is NOT a request for Quotes or Proposals, the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. The Northern Indiana Health Care System (NIHCS), Ft. Wayne, IN, is conducting a market survey for qualified firms capable of providing the required hotel/motel services as outlined below. Applicable NAICS Code is 721110, size standard $30.0M. Responses to this notice MUST be submitted in writing (email or fax is acceptable) and received no later than September 2, 2011. Please provide your Company Name, Point of Contact, DUNS Number, Email, and Socio-Economic Category of your business. No telephone inquiries will be accepted. STATEMENT OF WORK GENERAL:Contractor shall furnish VA Northern Indiana Health Care System Homeless Veteran services consisting of emergency housing to Veteran beneficiaries for who such care is specifically authorized. This contract will consist of a base year, with one (1) renewal option for an additional year to be exercised at the discretion of the government. The contracted services will be paid regardless of occupancy. SCOPE OF WORK 1.Contractor shall make available five (5) rooms per night for Veterans referred by VA Northern Indiana Health Care System. These rooms will be paid whether occupied or not and will be located in Fort Wayne or Marion, IN. 2.Each Veteran referred under this contract will be screened for eligibly by the VA Homeless Team Point of Contact prior to admission to the emergency hotel placements. 3.The contractor shall furnish emergency housing and the only cost should include standard practices of the hotel industry such as complimentary daily changing of linens, toiletries and other standard complimentary practices provided to hotel guests as part of the standard commercial practices of the Contractor. 4.Rooms must be tax free. 5.All amenities shall be the responsibility of the patient (i.e. room service, phone calls, pay-per-view movies, etc). The VA shall not be responsible nor pay for any damages caused by the patient. REQUIRED SERVICES: 1.Contractor shall be in compliance with the Americans with Disabilities Act, the Hotel and Motel Fire Safety Act of 1990 and must be listed on the U.S. Fire Administration Hotel and Motel Master found at: http://www/usfa.fema.gov/hotel/search.cfm 2.Contractor must be located within ¼ mile of the public bus transportation line in Fort Wayne or Marion, IN. 3.All five rooms are required 7 days per week. Of these rooms, the following "types" must be provided: a.Multiple-occupancy rooms (to accommodate families up to five individuals). Rooms must have two (2) queen beds and have the availability of a cot/roll-a-way bed if needed. b.One Handicap Accessible room per week c.One Smoking room per week d.One Room that will allow for pets. 4.Doors opening to outside areas shall be weather tight. 5.Contractor shall provide rooms with a refrigerator and microwave to assist in food storage and preparation. 6.All rooms shall be heated and air conditioned to be able to be maintained at temperature between 68-72 degrees Fahrenheit. 7.Contractor shall provide handicapped accessible parking. 8.No subcontracting will be allowed for this solicitation.
