Description
RECOVERY - PER FAR 5.7 THIS NOTICE IS PROVIDED FOR INFORMATION PURPOSES ONLY; THEREFORE FAR 5.203 DOES NOT APPLY. THIS OPPORTUNITY WAS AVAILABLE ONLY TO CONTRACTOR(S) UNDER THE CURRENT CONTRACT NUMBER(S): DEMARIA BUILDING COMPANY INC. W911XK-10-C-0003 VA HVAV AT SAGINAW, MICHIGAN A. SCOPE OF WORK DT057-1 Audiology Booths The purpose of this change is to incorporate Government directed changes into the contract by a multiple part modification, with this change being part 1. Pursuant to FAR Clause 52.243-4, CHANGES, the Contractor is directed to proceed with the following: A. The contractor shall delete the requirements to provide patient and staff access to the 4th floor audiology suite and after administrative hours only in the 4th floor audiology suite from the current contract. This does not remove the requirement to provide patient and staff access from the passenger and service elevators to 4th floor east wing. B. The contractor shall provide and install 2 temporary sound booths per sketches DT057-SK1 and DT057-SK2, 1 each in rooms 1-4115 and 1-4101, and provide power to the booths (for lighting and ventilation system). C. The booths shall be 5'x7' double wall isolation with a minimum of the following: Standard ventilation system Interior duct covers Wireless remote Light Cable passages Studio foam Wide access door window (16"x48") Window (26"x46") Window (26"x46") with 10 audio jacks (2 RCA jacks and 8-1/4" Stereo jacks) D. The contractor shall remove the booths after audiology moves back into their permanent space. The contractor shall provide a storage container for storage of the sound booths for the VAMC's future use. The power for the temporary booths shall be demolished to the neares junction box and indicate in the panel as a spare after usage. The contractor shall not exceed Ten Thousand dollars ($10,000) in performance of this scope with prior written direction from the Contracting Officer as a 2nd part contract modification. The contractor shall submit a proposal within 7 calendar days of this contract modification.
