Description

CONTRACTOR agrees during the term of this agreement to provide digital stereotactic breast biopsy equipment in a mobile configuration; a certified Radiologic Technologist experienced in stereotactic breast biopsies, and a female assistant on a pre-arranged mutually acceptable schedule. CONTRACTOR agrees to provide hard copy documentation of each stereotactic biopsy performed. CONTRACTOR shall provide, at no cost to VALLHCS, copies of all quality assurance and licensure documentation necessary to maintain VALLHCS accredited mammography program. VALLHCS agrees to provide suitable, secure site and power source for the mobile system. VALLHCS agrees to provide accredited physicians to perform the procedure, interpret the clinical results, and such support staff as may be necessary. VALLHCS agrees to schedule a minimum of one (1) stereotactic biopsy per mobile site visit or pay a minimum charge of $1,000.00 per visit. CONTRACTOR agrees during the term of this agreement to provide portable Hand Held Mammotome ultrasound breast stereotactic biopsy equipment and an experienced female technologist and a female assistant on an as-needed basis. Services will be available from the Contractor within 10 business days (vs 14 calendar days) of notification by the VALLHCS. Services will be available Monday-Friday, exclusive of Federal holidays, 8:00am - 4:30pm. The Contractor will obtain consent for procedures. VA will also obtain consent with VA ICF in Imaging Service. The VALLHCS or other VA facility will provide follow-up outpatient care as indicated. The Contractor shall designate one person who can be contacted for any clinical problems or issues that arise in the performance of this contract. The designee for the Government is the Contracting Officer Technical Representative (COTR) for this contract and will be identified after award. The Contractor will also have current malpractice insurance. The parties agree that the personnel working for the Contractor will not be considered employees of the VALLHCS for any reason and shall be considered employees of the Contractor. The Contractor will ensure the health (up to date TB skin testing), competency and current credentials and privileges of their personnel. The Contracting facility must be accessible and convenient for both wheelchair and ambulatory patients. The contractor will use VALLHCS power supply. Any proposed change in the availability of a service will be reported 14 days prior to any procedures.
