- AMENDMENT 001 BEGINS The purpose of this amendment is to add additional work and respond to questions and clarifications raised in the Site Visit conducted August 15. A list of questions and additional information follows. The closing date and time for submission of proposals is unchanged. Proposals must be received not later than 3:00 pm Thursday, September 1, 2011. Offerors must acknowledge receipt of this amendment in Block 19 of the SF 1442 submitted with their proposal. POC for additional questions is Kay Crowthers at 612.344.2163 or Kathryn.Crowthers@va.gov. CLARIFICATIONS 1.The police desk area with furniture systems remains as is. Any relocation of equipment at the police desk will be done by the government. The electrical connections for police systems and monitors run through the walls and should not be impacted by the new door. 2.All cabinets and counters are government furnished, contractor installed. 3.VCT in the water area is acceptable. QUESTIONS 1.Does the new door to the police desk require a key pad? ANSWER: No. The contractor shall provide standard hardware and the VA will provide and install the core. 2.The new wall will change the security camera coverage. Who is responsible for relocating security cameras? ANSWER: The contractor. One security camera shall be relocated approximately 10 , depending on final coffee shop design. Modify CAT5 and power cabling as required. 3.Will the VC registration desk be relocated? ANSWER: Yes that space will become the new Urgent Care waiting area. 4.Does the Equipment list include a dip well? ANSWER: No. Item not required. 5.Is there any flexibility in location? Can the site be moved away from the police desk? A 42 gap could potentially reduce the cost. ANSWER: No. The site must remain adjacent to the police desk. It may be moved forward or backward several feet but not side to side. ADDITIONAL WORK 1.(This item to be included in the base price.) Flooring: Contractor shall provide new carpet tile in urgent care waiting area around back of coffee shop; see shaded area of drawing. The actual amount will be dependent on the final layout of the coffee shop, but will be approximately 170 square feet. Carpet selection shall be approved by VA interior designer. The coffee shop area shall have VCT flooring. 2.OPTION 1 (provide separate pricing): Contractor to provide a security gate to be installed at front of the counter. Security gate shall be the full width of the opening in front of the counter. Assume a coffee shop width of 16 feet. Security gate shall have a metal ceiling track. The gate shall be full height from floor to the ceiling. Gate shall be bi-parting and meet in the middle of the opening. Provide locking mechanism keyed to BEST keying system to match existing VA Key system. System shall have pocket doors at both ends for storage of security gate system during hours of operation. Provide powder coat or aluminum finish. Finish to be selected from manufactures standard finishes. The security gate shall manually operated. Provide all materials, labor and equipment for the installation of the new security gate. Provide structural support and ceiling modifications as required. If selected, this replaces the requirement to install the VA provided security grille as noted on the proposed layout drawing. AMENDMENT 001 ENDS
