- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued; The provisions and clauses incorporated herein are those in effect through Federal Acquisition Circular 2005-23, effective June 12, 2008. The Department of Veterans Affairs Medical Center/Employee Education Resource Center, St. Louis, MO proposes to enter into a firm fixed price, competitive commercial. This solicitation is unrestricted and The North American Industry Classification System (NAICS) is 611430 and the small business size standard is $7.0 million. The purpose of this contract is to provide specific training for Veterans Health Administration (VHA) Amputation System of Care (ASoC) clinical care providers. The three programs requested will train our ASoC providers so that they will be able to provide the same training in the future to others within the the ASoC (train the trainer). After this year, it will be VA employees providing the training to other VA employees and to our Veterans with amputations. RESPONSES ARE DUE WEDNESDAY, August 31, 2011 by 4:00pm Eastern Time. E-Mail to: William.Singleton2@va.gov PERFORMANCE WORK STATEMENT 1. Title of Contract: Series of Amputation "Train the Trainer" Programs 2. Scope of Work: The contractor shall provide all labor, supervision and all other resources required to deliver Veteran Peer Visitation Program; Caregiver Peer Visitation Program; and Promoting Amputee Life Skills (PALS) Program. This is training will prepare Amputation System of Care (ASoC) providers to be able to provide the same training (train the trainer) in the future to others within the ASoC. 3. Background: The VA System provides care to over 42,000 Veterans with limb loss. The adjustment to disability following limb loss is difficult and requires skilled professionals to assist the person with limb loss and their caregivers in the process. The series of programs shall provide VA clinicians with new learning and additional tools to assist in the rehabilitation of amputees. Department of Veteran Affairs Secretary's Guiding Principles are: Veteran Centric, the programs are designed to directly impact the care provided to the individuals with limb loss and their caregivers; Results Driven, certification is required at the completion of the training in order to be able to train other employees; and Forward Looking, the project for the caregiver peer training is the first of its kind in the United States. 4. Period of Performance: This will be a series of 2-3 day sessions for the Peer Visitation Program, the Caregiver Peer Visitation Program and Promoting amputee Life Skills training. Periods of performance shall be between June 28, 2011and December 31, 2011. 5. Type of Contract: The Government anticipates award of a Fixed Price contract. 6. Place of Performance: Performance will be at the following government, medical facilities, Cleveland VA Medical Center (VAMC) Knoxville VAMC, Minneapolis VAMC, Richmond VAMC, Denver VAMC, Tampa VAMC, and Palo Alto VAMC. Hours of service - Contractor shall provide service during normal business hours from 8:00 A.M. to 4:30 P.M. local time, Monday through Friday. 7. Government Recognized Holidays: Unless specifically authorized in writing by the Contracting Officer, no services will be provided and no charges will be incurred and/or billed to any order on this contract on any of the Federal Holidays listed below. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the Contracting Officer has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. 9. GENERAL REQUIREMENTS For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. 10. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES The contractor shall perform the mandatory tasks and provide the specific deliverables described below within the performance period stated in this SOW. If for any reason, any deliverable cannot be delivered on time according to the below schedule, the contractor shall provide a written explanation to the COTR three (3) days prior to deliverable due date. This written transmittal shall include a firm commitment of when the work shall be completed. This transmittal to the COTR shall cite the reasons for the delay, and the impact on the overall project. Task 1. The contractor shall provide a detailed project management plan and briefing for the VHA project team which presents the contractor's plan for completing the contract. The contractor's plan shall be responsive to this SOW and describe, in further detail, the approach to be used for each aspect of the contract as defined in the technical approach. At a minimum, the project management plan shall include the detailed schedule, cost requirements, and proposed personnel. Deliverable 1.1: A detailed project management and work plan and kick-off briefing within 10 days after receipt of contract award. Task 1.2: The contractor shall provide instructor led training and materials for Veteran Peer Visitation; Caregiver Peer Visitation and Promoting Amputee Life Skills program training. Deliverable 1.2.1: Provide a copy of training materials for participants. Deliverable 1.2.2: Provide completion certificates for each participant. 11. EVALUATED OPTIONAL TASKS AND ASSOCIATED DELIVERABLES NA 12. FORMAL ACCEPTANCE OR REJECTION OF DELIVERABLES: The Government will review each deliverable and provide feedback/comments. The contractor shall have five (5) business days to incorporate feedback/comments and make appropriate revisions. The contractor shall provide the revised version of each deliverable to the COTR. The COTR will review and determine final acceptance by the Government. The COTR will notify the contractor of final acceptance within (5) five business days. 13. CHANGES TO THE SOW Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. 14. CONTRACTOR REPORTING REQUIREMENTS NA 15. TRAVEL NA 16. CONFIDENTIALITY AND NONDISCLOSURE 16.1. It is agreed that: 16.2. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this contract, are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the contract. 16.3. The Contracting Officer will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract, presented to the contractor, shall be submitted to the Contracting Officer for response. 16.3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer.
