- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, Purchase #FA4621-11-T-0041, is being issued as a Request for Quotation, which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-53. This acquisition is reserved for small businesses; however, the Government will accept proposals from all businesses. In the event of insufficient small business participation, award will be made on an unrestricted basis. The associated NAICS code for this acquisition is 561621 with a size standard of $12.5M. McConnell AFB KS plans to acquire the following items. 0001 - Manhole/Handhole Site Survey- Period of Performance: 7 days STATEMENT OF WORK Site Survey of McConnell AFB Manhole/Handhole locks 30 Sep 2011 McConnell AFB, Kansas 1. General: This Statement of Work (SOW) establishes the requirements to conduct a site survey of the manhole/handhole system at McConnell AFB. 2. DESCRIPTION OF SERVICES: There an estimated 300 manholes/handholes (in ground cable vaults) that need to be measured to determine what size of lockable device is required to secure assets within. The scope of work includes all manholes/handholes on the active duty base, Kansas Air National Guard (KANG) and base housing sides of base. The contractor shall provide all personnel, equipment, tools, materials, supervision and any other items and services necessary to accomplish the survey required to include the following: • Gas detection equipment • Blower to air out manhole/handhole • Manhole cage/Safety barrier • Confined Spaces Safety Plan The primary technical services shall be performed by qualified individuals who are experienced in confined spaces. Contractor must submit qualifications with quote. At the completion of site survey, the contractor shall provide documentation of results in electronic format. 3. PERIOD OF PERFORMANCE: 7 days. 4. WORK HOURS: 7:30 AM to 4:30 PM CST Monday through Friday, excluding Federal Holidays. Emergency technical support call hours are 24 hours a day, 365 days a year. AFFARS 5352.201-9101, Ombudsman (10 Aug 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
