Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 and FAR 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Quoters are responsible for monitoring this site for the release of any amendments or other pertinent information. This solicitation, D33N1511017, is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53. This acquisition is a total small business set-aside. The Contractor shall provide all travel, supplies, materials, and personnel to provide Speech-Language Services for Rocky Ridge Boarding School in Kykotsmovi, Arizona 86039 for School Year 2011-2012 for two days per week for 40 weeks (80 days total) in accordance with the Statement of Work. Statement of Work: Background: The Bureau of Indian Education (BIE), Navajo Nation, serves as a branch of the state education agency (SEA) for several elementary and secondary schools and dormitories serving the Navajo Reservation students located in the states of Arizona, New Mexico, and Utah. The Statement of Work calls for a Contractor to assist Rocky Ridge Boarding School in implementing the special education and related services as authorized under P.L. 108-446 Individualized with Disabilities Education Improvement Act (IDEIA). The purpose of IDEIA program is to improve academic achievement of students with disabilities and introduce strict accountability measures to hold schools, districts, and states responsible for the academic results of students with disabilities. Approximately 50 schools will be funded Part B Funds through a formula based process to provide special education and related services for eligible children with disabilities. Period of performance: Two days per week for 40 weeks (80 days) from date of award through July 31, 2012. Position: Speech Language Pathologist in the State of Arizona. Identified Students: The Speech Language services shall be provided in accordance with the identified student's Individualized Education Plan (IEP) which will specify the number of minutes per week or month and method of delivery. The estimated number of days for Speech Language Services for this school year is 80 days (2 days a week for 40 weeks). Scope of Work: The Contractor shall: 1. Evaluate and screen students due to have Speech/Language Evaluation or students due to have re-evaluation; 2. Prepare and provide ten (10) comprehensive evaluation reports; 3. Conduct Individual Education Plan (IEP) meeting for each student identified as eligible for Speech/Language service. During this meeting, the Contractor shall present and explain test results and therapy reports/services to the general education teacher, special education staff, and parents; 4. Provide and use their standardized testing instruments; 5. Provide direct speech/language therapy services in small groups or individual setting twice per week; 6. Generate a schedule along with the School's Special Education Teacher; 7. Provide quarterly technical assistance and training to para-educators, who will provide specified speech/language services to qualified students when the contractor is not present; 8. Provide speech/language in-service to the Special Education staff to explain diagnostic summary; 9. Provide Augmented Language and Assistive Technology/Speech training to staff; 10. Provide Speech/Language therapy and augmentative communication services according to IEPs; 11. Coordinate and maintain such record keeping as is necessary; 12. Comply with all federal mandates, State Board of Education regulations, and BIE policies for service to the disabled; 13. Maintain clinical service notes and working student file.14. Provide Arizona Certification and background clearance to Rocky Ridge Boarding School. Responsibilities of the Contractor: The Contractor shall: 1. Report to a designated time and place for orientation to the organizational structure for service provision and the procedures delineated for providing the above specified services;2. Make any additional reports or supply data to BIE which may be required to evaluate and report on the provision of related services;3. Make appropriate travel arrangements including payment for costs to be paid upon competition of the scheduled work at the Rocky Ridge Boarding School. Travel costs shall be reimbursed at the actual costs and in accordance with the Federal Travel Regulations;4. Provide written reports and other documentation of services i.e. evaluation forms, attendance sheets, etc. for specific services as requested by the BIE; and5. Develop and provide all materials for professional development and technical assistance provided to schools, leadership team, staff, and/or BIE. Instructions to Quoters: All Quoters shall submit a quote consisting of a separate technical and price volume. The volumes shall be as follows: Volume One: Technical Quote and Volume Two: Price Quote. Each volume shall be separate and complete in itself so that evaluation of one may be accomplished independently of evaluation of the other. a. Basis of Award The Government will make an award based upon best-value. Technical factors as a whole are more important than price. The evaluation factors are as follows in descending order of importance: 1. Qualifications and Experience of Proposed Personnel2. Technical Approach3. Past Performance b. Volume 1: Technical Quote The technical quote must discuss the proposed management approach in sufficient detail to clearly and concisely demonstrate that the Quoter has an understanding of all requirements specified in the Statement of Work. The technical quote shall not contain price information; however, resource information such as data concerning labor hours and categories, materials, subcontracts, etc. must be contained in the technical quote so that the Offeror's understanding of the detail and complexity of the requirements may be evaluated. b.1 Qualifications and Experience of Proposed Personnel The Contractor shall demonstrate that current and proposed staff has an appropriate range of relevant academic and applied experience as well as licenses in Arizona in Speech/Language Pathology. The technical quote shall contain resumes of the proposed personnel along with a copy of their applicable licenses. b.2 Technical Approach The Contractor shall describe its technical approach to accomplish each of the tasks delineated in the Statement of Work. b.3 Past Performance The Contractor shall provide recent and relevant contracts for the same or similar items, contract numbers, points of contacts with telephone numbers, and other relevant information. When evaluating past performance, the Government will focus on the areas of Quality of Service, Timeliness of Performance, Price/Cost Control, and Customer Satisfaction. Quoters should note that Past Performance related to the "quality" and how well a Contractor performed the services under a contract. c. Volume 2: Price Quote The Quoter shall prepare a firm fixed price quote that contains all information necessary to allow for a comprehensive evaluation of the costs proposed. The price quote shall include unit and total for the following: Date of award through July 31, 2012: 0001: Speech-Language Services Days Total Cost($___/hr) x 7 hrs/day = $_____/day) x 80 days = $_________(2 days/week x 40 weeks) 0002: Comprehensive EvaluationsReports$______/report 10 =$_________0002: Travel (Not-to-Exceed)Days (_____ miles x $.51/mile)80 days = $_________ TOTAL: CLIN 0001-0003 =$_________ The Contractor shall provide a complete explanation for each item detailing the rationale used in developing the cost. In addition, the Quoter shall submit supporting documentation that will facilitate the determination of whether the amounts proposed are fair and reasonable, e.g., vendor quotes for commercial-off-the-shelf products or services. These items are critical, as the Government will evaluate the Quoter's quote for reasonableness and completeness/accuracy. An analysis will be done on a "total price" basis consistent with a fixed price methodology, counting base and option periods as inclusive of the total fixed price. d.Question Submission: Questions must be submitted electronically via e-mail to Cathleen.Fraser@bia.gov in Word or compatible read-only format. Quoters shall also reference the RFQ number in the subject line of their quote. Quoters shall also reference the RFQ number in the subject line of their quote. The RFQ Due date (closing date) for quote submission is on or before 4:00 pm Eastern Time, Tuesday, August 30, 2011. f. Basis for Award: Award will be made the Quoter whose quote contains the combination of evaluated factors offering the best overall value to the Government. This will be determined by comparing differences in technical merit with differences in price to the Government. In making this comparison, the Government is more concerned with obtaining superior technical merit. However, the Government will not make an award at a significantly higher cost to the Government to achieve only slightly superior technical merit. SPECIAL PURCHASE ORDER REQUIREMENTS a. Background Investigation: In accordance with 25 U.S.C. 3201 (P.L. 101-630), Indian Child Protection and Family Violence Prevention Act; 42 U.S.C. 13041, Crime Control Act of 1990; 441 Departmental Manual, Personnel Security and Suitability Requirements all positions with duites and responsibilities involving regular contact with or control over Indian children are subject to a background investigation and determination of suitability for employment. Accordingly, all such positions require a favorable screening (applicant and suitability) determination made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program; and a favorable background investigation determination conducted by the Office of Personnel Management (OPM). A favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of the Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award or made within 60 days of incorporation of this clause into a current/existing contract. Pending completion of a background investigation by OPM, the Contractor who has been granted a favorable screening (applicant and suitability) determination must be within the sight and under the supervision of a Government employee who has had a completed background investigation (42 U.S.C 13041 (3)) during contract performance until a final background investigation determination has been made. Prior employment with the Federal Government, including BIA, OIEP or the Bureau of Indian Affairs, does not assure that the appropriate screening or investigation was conducted nor does it alleviate or guarantee a favorable screening (applicant and suitability) determination or background investigation. Prior to purchase order award or during the 60 day period discussed in paragraph two, it is solely the company, individual or contractor's responsibility to contact the BIA, OIEP Personnel Security and Suitability Program at 505-563-5287/5288/5289/5294 in order to obtain a complete security package, which includes but is not limited to appropriate investigative forms and/or inquire about their previous investigation; and, obtain a screening (applicant and suitability) determination, and subsequent final background investigation. If an unfavorable screening (applicant and suitability) determination is made, a purchase order shall not be awarded to the individual who received an unfavorable screening or to the company who has proposed to employ the individual to perform the work. The individual and or the company who proposed to employ this individual may be determined to be non-responsible if an unfavorable screening (applicant and suitability) determinations is made, which will make them ineligible for purchase order award. If an unfavorable final background investigation determination is made, the purchase order may be terminated and the employee in question shall not be authorized to provide service to the BIA, OIEP, if the services involves contact with or regular control over Indian children. b. Travel: The Contractor will be reimbursed for travel to provide support at a Government site or other site as may be specified and approved by the COTR under this effort. All travel shall be approved, by the COTR, prior to commencement of travel. The Contractor shall be reimbursed for actual allowable, allocable, and reasonable travel costs incurred during performance of this effort in accordance with the Federal Travel Regulations in effect on date of travel.The Contractor shall provide supporting documentation and a detailed breakdown of incurred travel costs with each invoice. CLAUSES: This solicitation incorporates one or more solicitation provisions by reference with the same force and effect as if they were given in full text. Upon request, the CO will make their full text available. The provisions at FAR 52.212-1 Instructions to Offerors-Commercial Items (June 2008) and FAR 52.212-3 Offeror Representations of Certifications (Aug 2009) apply to this acquisition. Quoters must include a complete copy of the provision FAR 52.212-3, Offeror Representations and Certification/Commercial Items with their quote.
