Description
Performance Work Statement This is a Centers for Disease Control and Prevention (CDC) combined synopsis/solicitation 2011-N-13301 for services that shall be provided by states due to the proprietary nature of the services. This announcement constitutes the only solicitation; proposals are being requested in accordance with the FAR Part 12 Clauses below. The Government contemplates award of a Firm Fixed Price contract with streamlined terms and conditions in accordance with FAR Part 12 and under authority of the Public Health Service Act. The data and records needed for this project are State owned, governed by State laws and regulations, and produced through State operated registration systems. This highly specialized data is available from no other source than the identified State or Territory. Each registration area has, by law, the exclusive authority to collect and record the events, and currently do so as a governmental function. As the sole entity authorized to and engaged in the collection of this information, each state agency is uniquely and solely capable of providing the requisite data to the Government. The NAICS Code is 541511. The purpose of this contract is to obtain a limited set of existing, individual level data from a minimum of three states. This data will be used to determine ways to improve the quality and completeness of Early Hearing Detection and Intervention (EHDI) data at the national level and help address questions related to assessing progress towards national EHDI benchmarks. The information in a limited data set (LDS) is not directly identifiable and excludes direct identifiers, such as names or social security numbers, of the individual or of relatives, employers, or household members of the individual. The data to be obtained is information that already exists and offeror's will not be required to collect new information, nor will they be required to develop a tracking system. The expected deliverables are: • Individual level data files are provided to the CDC EHDI Team with a defined set of fields that represent four consecutive quarters of data. • Names and other specified identifiers have been removed and the data in the transmitted data files have successfully undergone pseudonymization. • Reported data files have been verified for completeness and accuracy by the contractor and CDC • Issues related to the preparation and transmission of the data files have been successfully resolved Individual level data is available for evaluation from the Pilot Individual Early Hearing Detection and Intervention (iEHDI) project by approved EHDI Team staff at CDC. Following completion of this project in its entirety and any directly related analyses the submitted electronic data files in possession of CDC will be destroyed. This includes electronic and non-electronic copies of the data at the individual level. B. Background and Need The Centers for Disease Control and Prevention (CDC), National Center on Birth and Developmental Disabilities (NCBDDD), Division of Human Development and Disability (DHDD), EHDI program collaborates with federal, national, and state agencies and organizations in assisting states and territories to implement EHDI tracking and surveillance systems; assist states in enhancing EHDI infrastructure to improve their capacity to ensure that all infants are screened for hearing loss and receive appropriate follow-up services, as outlined in Goal 28-11 of Healthy People 2010; and maintain an information clearinghouse on issues related to EHDI that will be available through the CDC EHDI web site and teleconferences. Through these functions, the CDC EHDI program strives to ensure that newborns have the opportunity to communicate from birth. This program is a part of continuing national efforts to promote (1) early detection of hearing loss; (2) tracking of infants/children who are deaf or hard of hearing; and (3) the initiation of effective intervention systems. These efforts are essential roles of the CDC EHDI program. Part of the CDC EHDI program mission is to support states in the development of tracking and surveillance systems and collect and maintain a database with aggregate hearing screening and follow-up data. Although this aggregate level data has enabled the generation of national estimates of hearing screening, diagnosis and enrollment in intervention, there are several limitations and questions that cannot be addressed with this data, which is why more detailed, pseudonymized, individual data is needed. A full description of the EHDI re-authorization bill within the Public Health Service Act, authorizing statewide newborn and infant hearing screening, evaluation and intervention programs and systems, technical assistance, a national applied research program, and interagency and private sector collaboration for policy development, in order to assist the States in making progress toward the five goals, is located in Attachment F, with noted highlighted changes. Special Considerations The qualified offeror's should have experience in similar or related activities within the area of Early Hearing Detection and Intervention (EHDI), which include monitoring and tracking the disposition of every individual occurrent birth in the state. Qualified offeror's must provide signed letter of support from the person in charge of the EHDI program or the immediate supervisor of EHDI program staff. This letter must confirm that the data files, including at a minimum Tier 1 data items (Attachment A), can be transmitted to CDC and must be included with proposals. See the attached Statement of Work and Performance Matrix for the specific tasks, performance requirements and related attachments. This will be a performance-based contract. The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation -- Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. 6. Additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices can be found in the attached SF 18 solicitation document. INSTRUCTIONS TO CONTRACTORS Your response must be submitted in three parts: Technical Approach and Understanding of the Requirements, Organizational Capabilities, Personnel and Management Plan. Each part shall be separate and complete in itself so evaluation of one may be accomplished independently of the other. Please also include one (1) Compact Disc (CD) containing an electronic copy of the three parts in a Portable Document Format (.pdf). Evaluation Criteria Award: It will be made as a result of "best value" source selection. Best value means that the Government will perform a price/technical trade-off analysis such that business judgment will be exercised in selecting the most advantageous alternative to the Government, considering both the price and technical merit of the proposals. The Government will not make an award at a significantly higher overall price to the Government to achieve only slightly superior performance. The Government will make this assessment through the development of trade-off analyses and other analytic studies that involve the assessment of the benefits of the technically superior features. The determination of best value will be made by comparing the differences in the value of the technical factors with the differences in the prices proposed. Therefore, technical merit will be significantly more important than price. Technical Evaluation: Technical evaluation criteria for this contract are as follows: Criteria Points Technical Approach and Understanding of the Requirements 40% Organizational Capabilities 35% Personnel and Management Plan 25% Total Score 100% Technical Approach and Understanding of the Requirements Provide a detailed and comprehensive statement of the problem, scope, and purpose of the project to demonstrate complete understanding of the intent and requirements of the contract and potential problems that may be encountered. Describe the proposed technical approach to comply with each of the requirements specified in the Statement of Work. The proposal must be consistent with the stated purpose. The proposed approach must ensure the achievement of timely and acceptable performance and will include a milestone and/or phasing charts to illustrate a logical sequence of proposed events. Discuss the plan and solutions for overcoming difficulties involved with performing the work requirements. This criterion should include: • Availability and completeness of individual level child data • Annual birth cohort with minimum of 10,000 annual births • Available data, including access to Vital Records data and state level Early Hearing Detection and Intervention program data Organizational Capabilities Describe management's commitment of adequate and qualified resources to complete the tasks specified in the Statement of Work. Document recent successful experience in managing similar or related work that is comparable in scope. Discuss corporate experience in acquiring, retaining and managing the best-qualified staff. Personnel and Management Plan Provide a list of proposed personnel stating their experience, education, skills, job titles, and qualification to do the job. The backgrounds of the personnel will reflect the length and variety of experience and expertise in tasks similar to the tasks required by this project and any relevant training. If subcontractors are proposed, provide information to support their qualifications as well. Document the decision-making authority of the project director as related to other elements of the organization. The percentage of time each staff member will contribute to the requirements will be adequately identified. The extent to which outside consultants or staff from other departments, agencies, and/or organizations will be used and evidence of availability to be indicated. In addition, if staff from other departments, agencies, and/or organizations will be assisting in the work specified in this requirement the proposal should include letters of support indicating that these staff have official authority to provide data and will be able to provide this work.
