Description
Price and Cost Schedule for Quarterly PM, Repair Service and Parts. Wage Determination Contract Clauses Sterilization Equipment This acquisition is Total Small Business Set-Aside, restricted under NAICS code is 811219 and a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for Quotation (RFQ) are being requested and a written solicitation will not be issued. The current solicitation number is 11-247-SOL-00064 and is issued as a RFQ. This acquisition is being conducted under Simplified Acquisitions Procedures in accordance with FAR part 13. The clauses and provisions referenced in this solicitation may be obtained in full text form at http://www.arnet.gov/far. The Department of Health and Human Services (DHHS) /Indian Health Service (IHS) has a requirement for preventive maintenance, inspection, and certification and/or parts and repairs (if required) for the Preventive Maintenance of Sterilization Equipment for the Hopi Health Care Center in Polacca, Arizona 86042 Contractor shall be required to provide trained personnel with a minimum of at least 5 years experience in the preventive maintenance, inspection, and certification for sterilization of equipment. The contract will be a firm fixed price and is expected to include a base year from the date of award with four (4)-one (1) year options to extend. The solicitation will be issued on or about August 17, 2011 with an anticipated proposal due date of August 31, 2011. All questions must be in writing and may be e-mailed to Felicia.aspaas@ihs.gov. Offeror is responsible for downloading all documents including amendments, without further notification from the Contracting Officer. Interested firms responding to this Total Small Business Set-Aside and Combine Synopsis/Solicitation must adhere to the following: (a) Provide a capability statement demonstrating relevant experience, skills and ability to fulfill the Government's requirements for the above. The capability statement should contain enough sufficient detail for the Government to make an informed decision regarding your capabilities; however, the statement should not exceed 3 pages. (b) The capability statement must identify the responder's: small business type and size; DUNS number; NAICS code; and technical and administrative points of contact, including names, titles, addresses, telephone and fax numbers, and e-mail addresses. (c) All capability statements must be submitted electronically no later than 4:30 pm mountain standard time on Wednesday, August 31, 2011 to Felicia Aspaas, Felicia.aspaas@ihs.gov A. Sterilization Equipment Preventive Maintenance Contract I. STATEMENT OF WORK: The Contractor shall provide quarterly preventive maintenance, inspection, certification, tools, and/or parts and repairs (if required), including all associated travel costs for the Sterilization Equipment for the Hopi Health Care Center (HHCC) in Polacca, Arizona 86042. The contractor will perform preventive maintenance, inspection, and certification for the Steris Sterilization Equipment as identified below; • Prevac Sterilizer: CE TAG # OTR2150, Steris, M/N: V-120,S/N: 0135199-06/ Steris Routine Maintenance Manual P/N P-129367-410 • Sonic Console: CE TAG # OTR2153, Steris, M/N: SC1224CD, S/N: 0433499058/ Steris Routine Maintenance Manual P/N P-471162- • Washer/Disinfector: CE TAG # OTR2152, Steris, M/N: 444, S/N: 3630699004/ Steris Maintenance Manual P/N P-764329-133 • Warming Cabinet: CE TAG # LND2006, Steris, M/N: QDJ02, S/N: 04301990/ Steris Maintenance Manual P/N P-129361-177 • Warming Cabinet: CE TAG # OTR2184, Steris, M/N: QDJ02, S/N: 0430599009 / Steris Maintenance Manual P/N P-129361-177 A. PREVENTIVE MAINTENANCE (PM) Preventive maintenance procedures shall include but are not limited to the following: a) Cleaning of equipment; return equipment in a clean state after work is completed. b) Completing Original Equipment Manufacturer (OEM) field service updates for operational and reliability. c) Performing quarterly in conformance with manufacturers and with the OEM d) Performing remedial maintenance of non-emergent nature. e) Testing and replacing faulty and worn parts. f) Inspecting/replacing electrical wiring and cables for wear and fraying. g) Inspecting all mechanical components, including, but not limited to: cables and mounting hardware, pumps, seals, belts, interlocks, sensors, controls, bearings, hoses, heaters, radiators, and motors for operational integrity, safety, and performance to OEM specifications. h) Preventive maintenance shall consist of replacing parts, calibration, cleaning, oiling, adjusting and maintaining the equipment except those services necessitated by accident, fire or abuse. i) Returning the equipment to operating condition defined in OEM specifications. j) Replacing any OEM labels, decals, or warning tags that are not legible. k) Providing documentation of service performed. PM Procedures: PM procedures shall be in accordance with the published preventive maintenance manuals for the equipment listed above (provided upon request). The Contractor shall utilize the Original Equipment Manufacturer's established procedures and checklists, or Contractor-supplied equivalent satisfactory to the designated Contract Officer Technical Representative (COTR). Equipment Certification of Inspection: The contractor shall label all equipment upon completion of service identifying at a minimum the date, time, company name and initials. Corrective Maintenance The Contractor shall notify the designated COTR of the existence or development of any defects or repairs to the equipment covered under this contract which the Contractor considers is not applicable to terms of preventive maintenance (such as operator misuse). Preventive Maintenance Scheduling: The contractor shall provide a preventive maintenance schedule to the COTR for approval which divides the PM workload into quarterly visits. Exceptions to the Preventive Maintenance Inspection Schedule shall be arranged and approved in advance with the designated COTR. Schedule and Notification: The contractor shall notify the designated COTR prior to beginning services and upon completion. Contractor shall comply with IHS visitation policy which may require obtaining a Contractor badge prior to beginning services. B. REPAIR SERVICE a) The contract includes repair however the COTR must approve the request for a repair service call from the Contractor. b) Response Time: The Contractor must respond with a phone call to the designated COTR within two (2) hours after receipt of notification in an effort to correct the problem by phone. If the problem cannot be corrected verbally, the Contractor shall commence work (on-site physical response) within twenty-four (24) hours after receipt of notification and shall proceed aggressively to completion without undue delay. C. PARTS: a) The Contractor shall provide all necessary tools, supplies and equipment to provide the required services. b) The Contractor shall maintain the equipment, covered by this contract, in accordance with the OEM. The Contractor stipulates that he/she has ready access to new standard parts (manufactured, supplied by the manufacturer, or equal thereto). All parts supplied shall be of correct manufacture and have full compatibility with existing equipment. Documentation of intended parts source(s) shall be provided to the Contracting Officer upon request. D. DOCUMENTATION/REPORTS: The Contractor shall submit a legible field service report, test reports, which shall include detailed descriptions of the preventive maintenance, test inspection, certification and/or repair service performed, including replaced parts and estimated prices required for the service call. E. CONFORMANCE STANDARDS: Contractor shall comply with Hospital Accreditation Standards, compliance with The Joint Commission (TJC), Original Equipment Manufacturer (OEM), Association for the Advancement of Medical Instrumentation (AAMI- ST79), Federal, State, and Government regulations such as National Fire Protection (NFPA) and Occupational Health and Safety Association (OSHA), other recommended maintenance and Safety Standard. F. OPERATING HOURS: The contractor shall provide all maintenance, inspection, certification and/or repair services during normal operating hours Monday through Friday from 8:00 am to 5:00 pm excluding federal holidays. The service schedule dates shall be agreed by the contractor and the designated COTR. G. PERIOD OF PERFORMANCE: a) Award date through one (1) year period. b) The contract will be one (1) year with four (4)-one (1) year options to extend. H. CERTIFICATION/LICENSURE: The contractor shall be authorized to provide trained personnel with a minimum of at least 5 years experience on Steris Sterilization Equipment. Documentation of this training and experience will be submitted with quote and will be provided to the Facility Engineer with the Hopi Health Care Center Equipment.
