This proposed contract action is used for Oral Surgery Clinic. These items are requested to replace CMF Plates & Screws used in the fixation of the mandible and maxilla, these plates and screws are needed to ensure that the OMS clinic has the plates and screws needed to effectively provide for patients that will present with various fractures or disorders of the mandible, maxilla, and mid-face, for Oral Surgery Department. The OMS clinic currently has a Stryker Universal CMF system which requires replacement CMF plates and screws. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is American Medical Depot. CAGE Code: 0XP01, NAICS: 339112. If another vendor of CMF plates and screws were selected the department will be forced to purchase new instrumentation to complement the system selected. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that American Medical Depot is the Authorized Distributor for these products. The OMS clinic currently has a Stryker Universal CMF system which requires replacement CMF plates and screws for Oral Surgery Department. IAW FAR 5.207(c) (15) (ii), all responsible sources may submit a quotation, which shall be considered by the agency. The Government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-53. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil.
