: VA-260-11-RQ-1430 Notice Type: Combined Synopsis/Solicitation Synopsis: Added: August 05, 2011 This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Under Statutory Authority 41 U.S.C. 253(c)(1) the Alaska VA Healthcare System (AVAHS), Anchorage Alaska, anticipates entering into a procurement for fitting, adjustments and necessary to fits for eyeglasses. (ii) The reference/solicitation number is VA-260-11-RQ-1430 and the solicitation is issued as a request for quotation (RFQ). The combined synopsis/solicitation will result in a contract and corresponding purchase order. (iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and (VAAR). (iv) The North American Industry Classification System (NAICS) code is 446130 (size standards is $19.0 million) and applies to this solicitation. (v) Contractor shall provide a fixed rate for eye fitting services located in and around the following: 1)Anchorage Bowl Area- From Houston to Sutton to Girdwood 2)Fairbanks Bowl Area - From Ester to Fox to Delta Junction 3)Kenai Peninsula Area - From Homer to Seward to Hope 4)Juneau Area- From Juneau to Ketchikan DESCRIPTION/SPECIFICATIONS/WORK STATEMENT Contractor(s) shall provide fitting, adjustments and supervision necessary to fit for eyeglasses, as specified within and requested by the Prosthetic and Sensory Aids Service (PSAS), COTR or the Contracting Officer for the Alaska VA Healthcare System (AVAHS), 1201 N. Muldoon Rd, Anchorage, AK 99504, for a period not to exceed 1 year beginning May 1, 2011. Proposal will be considered from all offerors who are currently established and regularly providing these services, who are financially responsible and must provide documentation that all personnel are qualified to perform these services. All offerors must meet or exceed the current Joint Commissioned Accreditation of Healthcare Organizations (JCAHO) standards or other comparable accrediting body and provide substantiating proof to that fact. 2.3.1 SERVICE AREA: The contractor(s) agrees to provide service to all beneficiaries authorized by VA who reside in the State of Alaska. 5)Anchorage Bowl Area- From Houston to Sutton to Girdwood 6)Fairbanks Bowl Area - From Ester to Fox to Delta Junction 7)Kenai Peninsula Area - From Homer to Seward to Hope 8)Juneau Area- From Juneau to Ketchikan Service will include: Storage of demonstration eyeglass frames Fitting of eyeglass frames Adjustment of eyeglass frames Faxing prescription with frame information to Vancouver VA Locked storage of Veteran information Good customer service 2.3.2 POINT OF CONTACT: A Contracting Officers Technical Representative (COTR) will be delegated in writing by the contracting Officer. The COTR will be the person(s) authorized, by the Contracting Officer, to place orders against this contract. The COTR will notify the contractor by fax or in writing of services required. Admittance into the DME program for VA beneficiaries under this contract will be via authorization from the COTR. The COTR will authorize admittance into the DME program based on a prescription written by a VA or VA authorized provider. 2.3.4 SERVICES: 2.3.4.1Contractor to furnish all necessary services for the fitting, and adjustment, for eyeglass service a.The Contractor will meet or exceed Joint Commission compliance standards in the cleaning and disinfecting of recovered Durable Medical Equipment (DME), as required by the VA in the recovery, deliver and storage process, i.e., Contractor will wash in detergent/disinfectant solution, LPH or equivalent, label with a tag indicating date of cleaning, and return for storage in a non-contaminated storage area for future re-issuance. Services are to be performed prior to placing in storage areas. 2.3.4.2Contractor will provide a monthly inventory of all government-owned equipment and will submit monthly, with invoices, to Chief, Prosthetic and Sensory Aids Service (CPSAS) (121), Alaska VA Healthcare System, 1201 N. Muldoon Road, Anchorage, AK 99504. 2.3.4.3Contractor will have on staff one (1) qualified specialist who the VA can contact Monday through Friday between the hours of 8:00 A.M. and 4:30 P.M. to see patients on a walk in basis. 2.3.6 RECORD OF TRAINING OF CONTRACTOR PERSONNEL 2.3.6.1 Training certificates will be kept by the Contract at their facility. Copies of these documents will be provided to VA. 2.3.6.2Contractor will document appropriate orientation programs for all employees involved in the fitting of eyeglass frames with patients. Etc. The program will be ongoing and documented. 2.3.6.3Contractor requirements: a.Contractor is responsible to develop and maintain written policies and procedures for licensure, certifications, competency evaluations, orientation and continuing education for employees and/or subcontractors that are appropriate to the scope of care and services provided under this contract. Copies of these documents will be provided to VA. 2.3.8 SERVICE AREA: The Contractor agrees to provide service to all beneficiaries authorized by the VA who reside in the State of Alaska. 2.3.9 CONFIDENTIALITY: The contractor shall insure the confidentiality of all beneficiary/patient information in accordance with the Privacy act. 2.3.10WORKING HOURS: 2.3.10.1Services shall normally be performed during normal working hours, 8:00A.M. to 4:30PM Monday through Friday, except federal holidays. 2.3.10.2 Work shall not be performed on a weekday which is a Government holiday, or which is a day observed in lieu thereof. U.S. Government holidays are as follows; New Years DayJanuary 1 Martin Luther King's Birthday3rd Monday in January President's Day3rd Monday in February Memorial DayLast Monday Fourth of JulyJuly 4th Labor Day1st Monday in September Columbus Day2nd Monday in October Veteran's DayNovember 11th Thanksgiving Day Last Thursday in November Christmas Day December 25th When a holiday falls on a Sunday, the following Monday will be observed as a legal holiday, when a holiday falls on a Saturday, the preceding Friday is observed by U.S. Government Agencies. 2.3.14QUALITY ASSURANCE: Contractor shall have an on-going quality assurance program designed to objectively and systematically monitor and evaluate the quality and appropriateness of patient care, resolve identified problems and pursue opportunities to improve patient care. The written plan shall delineate the objectives of the quality assurance activities, scope of the activities, activities for monitoring and evaluation, methods of implementation of the monitoring and evaluation activities, methods of reporting results, mechanisms for taking follow-up action, and responsibilities of staff for each activity of the quality assurance program. The objectives, scope, organization, and effectiveness of the Quality Assurance plan shall be evaluated at lease annually and be revised annually as necessary. A copy of this plan shall be provided to VA. VA specific outcomes will be provided quarterly to the VA. Quarterly information specific to VA patients will include patient satisfaction, infection control, occurrence reports, and VA specific measurement outcomes. 2.3.15EMERGENCY PREPAREDNESS PLAN: The Contractor shall have an emergency preparedness plan which must demonstrate the ability to provide continuing care and support in the event of an emergency which results in interruption of services. The Contractor shall provide 24-hour emergency services including replacement and/or back-up equipment when necessary. An adhesive label with the Contractor's name and telephone number shall be placed on the equipment in an obvious location so that the patient and/or caregiver will know where to call to report emergency malfunctions. 2.3.16SAFETY REQUIREMENTS: 2.3.16.1If contractor has determined the recipient of the DME to have limited English Proficiency (LEP) the contractor shall contract the COTR for further instructions at (907) 257-4930. 2.3.16.2The Contractor shall have a program designed to educate appropriate staff and patients in safety measures in the home to minimize hazards related to the care or service provided. Initial and ongoing written and/or verbal instructions shall be provided. Instruction shall include the following: basic home safety, safe and appropriate use of medical equipment, storage, handling, delivery and access to supplies, medical gases, as appropriate, and the identification, handling and disposal of hazardous materials and wastes in a safe and sanitary manner and in accordance with applicable laws and regulations. The patient's knowledge and performance of safety procedures shall be monitored continuously and re-education provided as needed. All education shall be documented. 2.3.16.3The Contractor shall have a system for reporting patient safety issues and documenting all accidents and injuries and for documenting safety hazards. All reported accidents and injuries shall be investigated by the Contractor. A follow-up evaluation shall be provided to the Chief, PSAS, or designee, within five (5) working days of investigation. The results of these investigations shall be considered when reviewing the quality and appropriateness of patient care. Equipment malfunction and serious injury or death associated with equipment shall be reported to the vendor and/or manufacturer. 2.3.16.4In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer or his/her designee may determine to be reasonably necessary to protect the lives of health of the occupants of the buildings. The Contracting Officer or his/her designee shall notify the Contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the condition to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails to or refuses to comply promptly, the Contracting Officer may issue an order stopping all or any part of the work and hold the Contractor in default. 2.3.17INFECTION CONTROL: 2.3.17.1The contractor shall take all necessary measures to prevent, identify and control infections. Infection control procedures related to the care or service provided shall address, at a minimum, personal hygiene, isolation precautions, aseptic procedures, staff health and transmitted infections, and appropriate cleaning and sterilization of equipment. Contractor staff shall be instructed in the importance of infection control and personal hygiene and know their responsibilities regarding infection control. Copies of infection control procedures shall be provided to VA. 2.3.17.2The Contractor shall have a system in place to evaluate report and maintain records of infection related to the care or service provided among patients and as appropriate, among staff. The Contractor shall provide documentation of training in infection control standards for all Contractor personnel utilized in the performance of this contract. A copy of the certification of the completion of specialized training by technicians shall be provided to VA. 2.3.18PROFESSIONAL BEHAVIOR: 2.3.18.1The following provision shall apply when the Contractor and Contractor representatives make contact with a beneficiary, family member, or representative acting on behalf of the beneficiary: a.Beneficiaries may have major physical disabilities or chronic illness that may influence their behavior and lifestyle. It is of the utmost importance that a high degree of professionalism be demonstrated when contact is made. The Contractor shall be discrete and tactful and show concern, compassion and patience. Patient safety and patient input into the care/treatment delivered shall be recognized. b.The Contractor shall assure that all representatives are well qualified to set-up, demonstrate and instruct the veteran and/or caregiver in the proper and safe use of the equipment. This will include instructions for normal routine maintenance to be performed by the user. Any questions related to decisions to repair or replace equipment or veteran benefits will be referred to the Chief, PSAS for a response. c.The VA reserves the right to request the removal of any Contractor personnel from performance of service under this contract if his/her behavior is not in accordance with the specifications contained herein. 2.3.19CONTRACTOR'S WRITTEN PROCEDURES FOR PERFORMING WORK: The Contractor must have established written procedures to deliver, recover, receive, clean and, when appropriate, sterilize reusable items. Such procedures must address the separation of clean and unclean equipment in storage, and delivery and recovery of DME items. All equipment must be labeled or tagged and dated to document such procedures. Copies of these procedures shall be provided to the VA. It is anticipate that a firm fixed price contract for a year (12 Months) will be awarded. "(vii) Delivery and acceptance of deliverables will be FOB destination. Period of performance is during 12 months as of the effective date of the contract. "(viii) The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation and incorporate the following addenda to 52.212-1, 52.204-6 Data Universal Numbering System (DUNS) Number; VAAR 852.219-70, 852.233-70 Protest Content; 852.236-76 Correspondence; 852.237-70 Contractor Responsibilities; 852.270-1. "(ix) The provision at FAR 52.212-2, Evaluation - Commercial Items applies to this requirement. The Addendum To 52.212-2 Evaluation - Commercial Items (JAN 1999) applies and any award(s) resulting from this solicitation will be made using the following method using cascade order of precedence: In accordance with FAR Subpart 19.14 and PL 109-461, award under this solicitation will be made on a competitive basis first to an eligible Service Disabled Veteran Owned small business (SDVOSB) concern (see, FAR 52.219-1, "Small Business Program Representations" for small business size definition) who submits a responsive offer, provided that there is adequate competition among such firms. If there is inadequate competition for award to a SDVOSB concern, award will be made competitively to an eligible Veteran Owned Small Business (VOSB) concern. If there is inadequate competition for award to a VOSB concern, award will be made competitively to an 8a or HubZone small business concern. If there is inadequate competition for award to an 8a or HubZone concern, award will be made competitively to a small business concern IAW FAR 19.5.. If there is inadequate competition for award to a small business concern, award will be made on the basis of full and open competition considering all responsive offers submitted by responsible business concerns. Adequate competition shall be deemed to exist if- "At least two competitive offers are received from qualified, responsible business concerns at the tier under consideration; and "Award will be made at fair market prices as determined in accordance with FAR 19.202-6. When there is inadequate competition at any tier, an otherwise competitive offer from a responsible business concern at that tier will be considered with the eligible offers for the next lower tier. For example, only one competitive offer is received from a SDVOSB business. That offer will be included with the offer(s) received from all VOSB businesses. If there is still inadequate competition, the offer from the SDVOSB business will then be included with SB offer(s) received; finally, if there is inadequate competition at the SB tier, the SDVOSB offer will be included with all responsive offers. Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and past performance considered. Pricing will be firm fixed price for each line item identified. The offeror(s) with the lowest cost/price alone will not necessarily be awarded the contract. Basis for contract award: This is a competitive best value acquisition in which competing offerors' past performance history will be evaluated on a basis significantly more important than cost or price considerations. By submission of its' offer, the offeror accedes to the terms and conditions of this solicitation, and all such offers shall be treated equally except for their prices and performance records. The evaluation process shall proceed as follows: Step 1: Initially, offers shall be ranked according to price. An Offerors quoted prices will be determined by multiplying the quantities identified in Schedule of Items by the quoted unit price for each Contract Line Item Number or Subcontract Line Item Number to confirm the extended amount for each. Technical factors are based on a pass/fail basis. Step 2: Using the no more than two references supplied by each interested contractor and the data independently obtained from other Government and commercial sources, the contracting officer shall seek performance information on the lowest priced proposal. The purpose of the past performance evaluation is to allow the Government to assess the Offerors ability to perform the effort described in this Request for Quote (RFQ), based on the Offerors demonstrated present and past performance. The assessment process will result in an overall risk rating of very good, satisfactory, neutral, marginal, or unsatisfactory. Offerors with no relevant past or present performance history shall receive the rating "neutral," meaning the rating is treated neither favorably nor unfavorably. In evaluating past performance the Government reserves the right to give greater consideration to information on those contracts deemed most relevant to the effort described in this RFQ. Step 3: If the lowest priced evaluated technically acceptable offer is judged to have a very good performance risk rating, then that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. The Government reserves the right to award a contract to other than the lowest priced offer if that offeror is judged to have a performance risk rating of "satisfactory" or lower. In that event, the contracting officer shall make an integrated assessment best value award decision The Government will award a firm fixed price contract with a base plus four (4) option year contract resulting from this solicitation. The evaluation factors for award are technical capabilities, past performance and price with technical capabilities and past performance being evaluated as significantly more important than cost considerations. "(x) The provision at 52.212-3, Offeror Representations and Certification, Commercial Items, applies to this solicitation. The contractor shall either: (1) return a completed copy of this provision with its quote (a copy of the provision may be attained from http://www.arnet.gov/far); or (2) complete the Online Reps and Certs. Application (ORCA) at http://www.bpn.gov and reference it in your quote. "(xi) The clause at 52-212-4 Contract Terms and Conditions - Commercial Items, applies to this acquisition.
