- estimated workload/volume SOW This is a combined synopsis and solicitation for Medical Laboratory Services for one base year plus four (4) option years. See attached full Statement of Work and estimated testing volume(med lab report 201106). The offeror must be able to perform all tests listed and provide pickup and delivery. Please quote firm fixed price for each test listed. If you have any questions/comments regarding the requirement, please submit via e-mail to below. Note: timeliness of services is non-negotiable. Offers will be evaluated on a best value basis and the most advantageous to the Government, considering price, technical capability, and past performance. Item 1: Medical Laboratory Testing Dates and Place of Delivery: The requested service shall take place between base year; 1 October 2011 - 30 September 2012 option 1; 1 October 2012 - 30 September 2013 option 2; 1 October 2013 - 30 September 2014 option 3; 1 October 2014 - 30 September 2015 option 4; 1 October 2015 - 30 September 2016. Hill Air Force Base, UT This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and therefore quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ) F2D0AD1144A001. Submit only written offers; oral offers will not be accepted.
