Description
Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, under Simplified Acquisition procedures in Subpart 13.1 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a small business set aside. The associated North American Industry Classification System (NAICS) Code is 813110 with business size standard $7.0M. It is the intent of the Government to issue two (2) fixed price purchase orders for a twelve(12) month base period and four (4) subsequent option periods. The solicitation number is 11-009034 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-49. The National Institues of Health has a requirement for two (2) contract chaplains for approximately 180/hrs per year to provide routine and on-call spiritual support services. At two points in the year, mid August through October, and mid April to mid May contract chaplain(s) are needed to assist the Clinical Center, Spiritual Ministry Department (SMD), Bethesda, MD. Services include; initial visits, routine follow-up/continual care visits, sacramental visits, referral visits, counseling sessions, care giver support and on-call services. The contract chaplain participates in interdisciplinary team meetings and patient care conferences as assigned. Additionally, the contract chaplain interprets the function and work of the SMD to the Clinical Center community and to the community at large. Offerors are referred to the attached Statement of Work for technical specifics, qualifications, and evaluation criteria. Interested parties who meet the requirements are invited to submit a response, including statements of how the proposal will meet the Government requirements. All information furnished shall be legible, typed and must contain sufficient detail to allow the government to evaluate and determine best value, see Evaluation Criteria-SOW. Technical and past performance will be considered for overall best value to the government. The hourly rate for the base and option periods if exercised is fixed at $30.00 per/hour. The solicitation incorporates the provisions at FAR 52.212-1, INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS-DEVIATION FOR SIMPLIFIED ACQUISITIONS, FAR 52.212-2, EVALUATION-COMMERCIAL ITEMS. Specific evaluation criteria is listed within the statement of work, titled Evaluation Criteria. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items-with duns number addendum, or, the offeror may provide documentation that the vendor is current and registered in Online Representations and Certifications Application (ORCA) on www.bpn.gov. The contractor must be registered in the Central Contractor Registration prior to award (www.ccr.gov). In addition, the resulting contract will incorporate the requirements of the clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, and the requirements contained in paragraphs (a) and (d) of the clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items-deviation for simplified acquisitions. The contract will also incorporate the following FAR clauses cited in paragraphs (b) and (c) of the latter clause: FAR 52.222-26, EQUAL OPPORTUNITY; FAR 52.222-35, AFFIRMATIVE ACTION FOR SPECIAL DISABLED AND VIETNAM ERA VETERANS; FAR 52.222-36, AFFIRMATIVE ACTION FOR HANDICAPPED WORKERS; FAR 52.222-37, EMPLOYMENT REPORTS ON SPECIAL DISABLED VETERANS OF VIETNAM ERA; 52.223-6 Drug Free Workplace; 52.235-33, payment by Electronic Funds Transfer-Central Contractor Registration (31 U.S.C. 3332); 52.217-8 Option to Extend the Services, 52.217-9 Option to Exercise the Term of the Contract; FAR 52.232-19 AVAILABILITY OF FUNDS, applies to this requirement.
