Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for quotation (RFQ) and all documents in response to this announcement must include solicitation number VA-258-11-RQ-0522. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-52. This solicitation is unrestricted for NAICS code 611310 with a small business size standard of $7.0M. The offeror shall submit pricing for the following Line Item: 1 1.00 LT ___________________ ____________________ Training fee for the " Chronic 'Disease Self Management(CDSMP) Master Trainer Program", Aug 8-12, 2011, to be held in Albuquerque, NM. Fee includes all training, material, deliverables and travel in accordance with the Statement of Work. ____________________ GRAND TOTAL --- ==================== The following is the statement of work that applies to this solicitation and the resultant contract. 1.SCOPE OF WORK: The contractor shall independently provide a 4 ½ day Chronic Disease Self-Management Program (CDSMP) Master Trainer course for 14 VISN 18 staff members and 7 Veterans. The contractor shall provide each participant who successfully completes the training with a Master Trainer Authorization Agreement that certifies them to train CDSMP Leaders. 2.Background: New Mexico VA Healthcare System is seeking to procure a 4 ½ day CDSMP Master Trainer training course for 14 VISN 18 staff members and 7 Veterans who work in the areas of Veteran Health Education, Health Behavior, Health Promotion and Disease Prevention, and selected Veterans. These 21 staff members and Veterans will become CDSMP Master Trainers in each of their facilities (Albuquerque, Amarillo, Big Spring, El Paso, Phoenix, Prescott, and Tucson) in VISN 18 in order to facilitate the "train the trainer" sessions for the CDSMP workshops conducted for VISN 18 Veterans who have chronic diseases. The contracted subject matter experts will certify VHA participants to become "Train-the- Trainer" certified as CDSMP Master trainers. In turn, they will impart the knowledge, skills and abilities in CDSMP to CDSMP Leaders. Contractor is required to provide services in order to meet identified needs set forth by the conference planning committee and VISN 18 needs. Chronic Disease Self-Management Program (CDSMP) provides self-management education for all patients with chronic disease and helps preserve functional ability while reducing costs. VISN 18 has no standardized CDSMP for patient health education that is consistently used across the VISN. By implementing a standardized CDSMP across the VISN, it is anticipated that health care costs will be reduced and improve staffs participation in Veteran Centric Care. The C&A requirements do not apply and a Security Accreditation Package is not required. 3.General Information/Objectives: In collaboration with the Planning Committee for the "Implementation of Chronic Disease Self Management Program": "provide a highly interactive presentation using adult learning/accelerated learning techniques based on "CDSMP Master Trainer" course "certify participants as Master Trainers in CDSMP; enable participants to train CDSMP leaders within VISN 18 4.Deliverables: The contractor shall provide all labor, supervision and other resources required to facilitate the "CDSMP" workshop. 4.1.Provide two CDSMP "Leader" and "Master Trainer" manuals on CD to NMVAHCS Veteran Education Coordinator. 4.2.Assign and provide names of instructors. 5.Period of Performance: The period of performance is August 8-12, 2011. Place of Performance: Work shall be performed at a site to be determined in Albuquerque, New Mexico. 6.Type of Contract: The Government anticipates award of a Firm Fixed-Price contract. A.CHANGES TO THE PWS Any changes to this PWS shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. B.CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1.The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this contract, are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the contract. 2.The Contracting Officer will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract, presented to the contractor, shall be submitted to the Contracting Officer for response. 3.Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer. The specific evaluation criteria to be included in paragraph (a) of that provision is as follows: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers and are listed in descending order of importance: 1.Technical: Offeror shall submit evidence of license and copyrights to the Chronic Disease Self-Management Program (CDSMP) Master Trainer Course 2.Past Performance: Offeror shall submit a minimum of two references (name of agency, point of contact, phone number, email address, fax number, date training was performed, and dollar value of contract) where the CDSMP Master Trainer Course has been taught within the last 3 years in either a private or Government agency. 3.Price: Price will not receive a rating however it will be evaluated for fair and reasonableness. Technical and past performance, when combined, are significantly more important than price. (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
